AMH、E2、LH、FSH水平评估女性不孕症患者卵巢功能状态的价值分析

Value analysis of AMH,E2,LH,FSH levels in evaluating ovarian function of female infertilities

目的探讨根据血清抗苗勒氏管激素(AMH)、雌二醇(E2)、促黄体生成素(LH)、促卵泡激素(FSH)水平评估女性不孕症患者卵巢功能状态的价值。方法选取2019年1~6月本院妇科收治的女性不孕症患者150例作为观察组,选取同期在我院进行健康体检的已育健康妇女150例为对照组,检测比较两组研究对象的血清AMH、E2、LH、FSH水平;通过绘制E2、LH、FSH水平及三者联合预测不孕的受试者工作特征曲线(ROC曲线),评估其预测不孕症患者卵巢功能状态的价值。结果两组研究对象的血清AMH水平比较差异无统计学意义(P>0.05);对照组研究对象血清E2、LH及FSH水平均显著高于观察组,差异有统计学意义(P<0.05)。E2、LH、FSH及三者联合预测不孕的曲线下面积(AUC)分别为0.897、0.938、0.902、0.980。结论不孕症患者的卵巢储备功能受损,E2、LH、FSH三者联合检测预测不孕的灵敏度及特异度最高,能更好地反映出女性不孕症患者卵巢功能状态。

Objective To investigate the value of serum anti-Mullerian hormone(AMH),estradiol(E2),luteinizing hormone(LH),and follicle stimulating hormone(FSH)levels in evaluating the ovarian function of female infertile patients.Methods A total of 150 female infertilities admitted to the gynecology department of our hospital from January to June 2019 were selected as observation group,and 150 parous healthy women underwent health examinations in our hospital during the same time were selected as control group.The E2,LH,FSH levels,and the three-level-combination receiver operating characteristic(ROC)curves predicting infertilities were drawn to evaluate its value of ovarian function in predicting infertilities.Results There was no statistically significant difference of serum AMH level between the two groups(P>0.05).The levels of serum E 2,LH and FSH in the control group were significantly higher than those in the observation group,and the differences were statistically significant(P<0.05).The area under the curve(AUC)for E2,LH,FSH,and the three-level combination predicting infertilities were 0.897,0.938,0.902,and 0.980,respectively.Conclusion The ovarian reserve function of infertilities is impaired.The detection of E2,LH and FSH combination in predicting infertility has the highest sensitivity and specificity,which can better reflect the ovarian function in female infertilities.
