31例青春发育延迟的病因分析及GnRH泵诊断价值的探讨 被引量:3

摘要 目的:分析31例青春发育延迟患儿临床特征及病因构成,初步探究短期促性腺激素释放激素(gonadotropin-releasing hormone,GnRH)泵在青春发育延迟诊断中的应用价值。方法:采用回顾性研究方法,分析31例青春发育延迟患儿的临床资料。GnRH刺激试验评价垂体对GnRH的应答能力。8例患儿采用了脉冲式GnRH泵刺激试验。随访所有患儿青春发育进展状况。结果:高促性性腺功能减低症有9例,低促性性腺功能减低症(hypogonadotropic hypogonadism,HH)19例以及男性体质性青春发育延迟(constitutional delay of growth and puberty,CDGP)3例。CDGP与HH患儿的就诊年龄、体重、身高、骨龄、睾丸容积和阴茎长度均无差异,基础和单次GnRH激发后黄体生成素(luteinizing hormone,LH)无统计学差异,1例CDGP患儿脉冲式GnRH泵刺激后LH达6.9 mIU/mL。7例HH患儿GnRH脉冲刺激3 d和6 d的LH水平高于单次GnRH刺激的LH水平,GnRH脉冲刺激3 d和6 d的LH水平无差别,表明短期脉冲式GnRH刺激3 d可恢复垂体对GnRH的应答能力。结论:短期GnRH泵刺激试验为鉴别CDGP和HH提供帮助,也为选择更为有效的脉冲式激素替代治疗提供依据。
