基于“SHA 2011”的安徽省老年人治疗费用分析

Study on the Treatment Cost for the Elderly in Anhui Based on SHA 2011

目的:分析安徽省老年人口治疗费用筹资、分配和使用情况及其特点,为安徽省健康老龄化等相关政策的制定和调整提供参考。方法:基于"卫生费用核算体系2011"(SHA 2011)的理论与方法,对安徽省老年人治疗费用进行核算与分析。结果:2017年安徽省老年人治疗费用为378.79亿元,占全省治疗费用的34.14%。治疗费用76.99%发生在医院,20.78%发生在基层医疗机构;65~69岁与70~74岁是老年人治疗费用消耗最多的两个年龄组,占比超过50%;在老年人治疗费用疾病分布中,居前五位的分别是循环系统疾病、呼吸系统疾病、肿瘤、损伤、中毒和外因某些其他后果和消化系统疾病;治疗费用主要来源于公共筹资和家庭卫生支出。结论:老年人治疗费用,尤其是住院费用已经给的社会和家庭带来严重的经济负担,要推进防治结合与医养结合工作,提升基层卫生服务能力,进一步优化筹资结构。

Objective:To analyze the circumstance and characteristics of fund raising,allocation and use of treatment cost for the elderly in Anhui Province,so as to provide reference to optimize relevant policies such as healthy aging policies of Anhui Province.Methods:Based on the theory and method of“Health Expense Accounting System 2011”(SHA 2011),the treatment cost of the el derly in Anhui Province was calculated and analyzed.Results:In 2017,the cost of treatment for the elderly in Anhui Province was 37.879 billion yuan,accounting for 34.14%of the total treatment cost in Anhui Province.76.99%occurred in hospitals and 20.78%in primary medical institutions;65~69 and 70~74 years old were the two age groups with the largest consumption of treatment cost,accounting for more than 50%among expenses.Among the distribution of diseases treatment of the elderly,the top 5 were circulatory system diseases respiratory system diseases,tumors,injury,poisoning and some consequences of external causes,and digestive system diseases.The funds mainly came from public financingand household health expenditure.Conclusion:The treatment cost of the elder ly,especially the hospitalization cost,has brought heavy economic burden to the society and families.It is necessary to promote the in tegration of prevention and treatment,as well as the combination of medical treatment and maintenance,to enhance the capacity of primary health services,and further optimize the financing structure.
