初诊多发性骨髓瘤患者血栓弹力图特点

Characteristic of Thromboelastogram in Patients with Newly Diagnosed Multiple Myeloma

目的 探讨初诊多发性骨髓瘤(MM)患者血栓弹力图(TEG)特点.方法 选取2015年8月至2019年7月江苏大学附属医院收治的29例初诊MM患者为观察组,28名健康体检者为对照组,进行TEG检测,并以分期、分型分析主要参数.结果 初诊MM患者R值、K值、MA值延长,α角、CI、EPL、LY30变小,与对照组比较,差异均有统计学意义(P<0.05).与I、II期MM相比,III期MM患者EPL、LY30显著变小,差异有统计学意义(P<0.05).IgG型MM患者K值较IgA型变小,差异有统计学意义(P<0.05).结?论 初诊MM患者存在凝血功能异常,随病程进展,高凝倾向可能升高、血栓风险可能增加.

Objective To investigate characteristics of thromboelastogram(TEG)in patients with newly diagnosed multiple myeloma(MM).Methods A total of 29 newly diagnosed MM patients admitted to the Hematology Department of Affiliated Hospital of Jiangsu University from August 2015 to July 2019 were selected as the observation group,and 28 healthy subjects were the control group.TEG of both groups was tested.The major parameters were further analyzed according to stage and classification.Results Rvalue,K value and MA of TEG in newly diagnosed MM patients were prolonged,whileαvalue,CI,EPl and LY30 were smaller,and the difference was statistically significant compared with the control group(P<0.05).EPL and LY30 of MM patients with stage III were significantly smaller,and the difference was statistically significant compared with MM patients with stage I and II(P<0.05).K value of MM patients with IgG type was smaller than that of IgA type,and the difference was statistically significant(P<0.05).Conclusion There was abnormal blood coagulation in patients with newly diagnosed MM.As the disease progresses,the tendency of hypercoagulability may increase and the risk of thrombosis may increase.
