气道护理管理对远距离院际转运机械通气患者不良反应的影响

Effect of Airway Nursing Management on Adverse Reactions of Long Distance Inter-hospital Transport in Patients with Mechanical Ventilation

目的探讨气道护理管理对远距离院际转运机械通气患者的不良反应的影响。方法回顾性分析本中心2008年5月~2018年5月完成的院际转运机械通气96例患者临床资料,根据气道滴水量分为对照组48例、观察组48例,两组均采取气道护理管理,对照组气道滴水量5ml/h,观察组气道滴水量10ml/h。比较两组院际转运不良反应、痰液粘稠度、转运情况。结果两组不良反应比较无统计学差异(P>0.05);转运前两组痰液粘稠度比较无统计学差异(P>0.05);转运后观察组患者痰液粘稠度评分低于对照组(P<0.05);运输过程中对照组管导管移位4例,观察组5例,比较差异无统计学意义(P>0.05);观察组运送成功率高于对照组,但比较差异无统计学意义(P>0.05)。结论对远距离院际转运机械通气患者采用气道护理管理能有效控制其不良反应,稀释患者痰液粘稠度,提高运送成功率。

Objective To investigate the effect of airway nursing management on adverse reactions of long distance inter-hospital transport in patients with mechanical ventilation.Methods Clinical data of 96 patients with inter-hospital transfer mechanical ventilation from May 2008 to May 2018 were retrospectively analyzed.Patients were divided into two groups according to the different humidification dosages,each with 48 cases.Control group received 5 ml/h,while the observation group received 10 ml/h.Then the adverse reactions,sputum viscosity and transporting situation of the two groups were compared.Results The incidence of adverse reactions showed no difference between groups(P>0.05).The sputum viscosity score showed no difference between groups before the transfer(P>0.05),which was lower in the observation group than in the control group after transfer(P<0.05).There were 4 cases of catheter displacement in the control group and 5 cases in the observation group during transportation,with no significant difference(P>0.05).The success rate of inter-hospital transfer in the observation group was higher than that in the control group,with no statistic difference(P<0.05).Conclusion The application of the airway nursing management for long distance inter-hospital transport in patients with mechanical ventilation can significantly control the adverse reactions,dilute the sputum consistency of patients,and improve the transport success rate.
