应用动态心电图诊断冠心病性心律失常的价值探究

Application Value of Dynamic Electrocardiogram to Diagnose Coronary Heart Disease Arrhythmia

目的探讨应用动态心电图诊断冠心病性心律失常的价值探究.方法回顾性分析2015年6月至2018年12月在广东药科大学附属第一医院心电图室检查的150例冠心病性心律失常患者的临床资料,均分别行常规心电图检查和动态心电图检查,将常规心电图检查作为对照组,动态心电图检查作为观察组,以冠状动脉造影作为金标准,对比两组在不同心律失常类型上的诊断率、诊断的敏感度、特异度和准确度.结果观察组在室性心律失常、房性早搏/短阵房性心动过速、阵发性心房颤动/心房扑动、Ⅰ-Ⅲ度房室传导阻滞等心律失常类型的诊断率明显高于对照组(P<0.05);观察组在敏感度、准确度上明显高于对照组(P<0.05),但特异度上两组无明显差异(P>0.05).结论应用动态心电图诊断冠心病性心律失常的价值显著,能有效诊出心律失常的不同类型,敏感度、特异度和准确度高,为临床医师的诊治提供了可靠依据.

objective To explore the value of application of dynamic electrocardiogram in diagnosis of coronary heart disease arrhythmia.Methods A retrospective analysis on June 2015 to December 2018 in our hospital the electrocardiogram examination of the clinical data of 150 patients with coronary heart disease arrhythmia,both received conventional electrocardiogram examination and dynamic electrocardiogram inspection.The control group received routine ECG examination,while the observation group received dynamic ECG examination.The coronary angiography was used as the gold standard.The different types of arrhythmia diagnosis,diagnostic sensitivity and accuracy,specific degrees of two groups were compared.Results The observation group in atrial arrhythmia,ventricular premature beat/short array room tachycardia,paroxysmal atrial fibrillation/atrial flutter,Ⅰ-Ⅲdegree atrioventricular block and other diagnostic rate of arrhythmia type was obviously higher than that of control group(P<0.05).The sensitivity and accuracy of the observation group were significantly higher than that of the control group(P<0.05),but there was no significant difference in specificity between the two groups(P>0.05).Conclusion Application of dynamic electrocardiogram in diagnosis of coronary arrhythmias is of significant value,and can effectively diagnose different types of arrhythmias with high sensitivity,specificity and accuracy.
