异丙酚复合瑞芬太尼麻醉对老年高血压手术患者血流动力学的影响

Effects of Propofol Combined with Remifentanil Anesthesia on Hemodynamics in Elderly Patients Undergoing Hypertension Surgery

目的研究异丙酚复合瑞芬太尼麻醉对老年高血压手术患者血流动力学的影响。方法回顾性分析2017年1月至2019年6月常熟市第五人民医院收治的高血压患者52例,按照麻醉方法的不同分为对照组和观察组,各26例。观察组实施异丙酚复合瑞芬太尼麻醉;对照组实施异丙酚麻醉。比较2组T1、T2、T3时间段的血压、心率,苏醒情况和药物使用情况以及不良反应发生率。结果2组收缩压、舒张压、心率在T1~T3时呈先下降后上升趋势,但变化不显著且组间差异无统计学意义(P>0.05);与对照组相比,观察组拔管时间缩短,OAA/S评分升高,艾司洛尔及硝酸甘油用量减少(P<0.05);观察组不良反应发生率低于对照组,(P<0.05)。结论异丙酚复合瑞芬太尼麻醉能够维持老年高血压手术患者血流动力学,改善苏醒质量,减少麻醉药物用量,降低不良反应发生率。

Objective To study the effect of propofol combined with remifentanil anesthesia on hemodynamics in elderly patients with hypertension.Methods A retrospective analysis was performed on 52 hypertensive patients admitted to changshu fifth people's hospital from January 2017 to June 2019,who were divided into control group and observation group according to different anesthesia methods,with 26 cases each.The observation group was anesthetized with propofol combined with remifentanil.The control group was anesthetized with propofol.Blood pressure,heart rate,wakefulness,drug use and incidence of adverse reactions in T1,T2 and T3 were compared.Results ystolic blood pressure,diastolic blood pressure and heart rate of the two groups decreased first and then increased at T1~T3,but the changes were not significant and the differences between the groups were not statistically significant(P>0.05).Compared with the control group,the extubation time was shorter in the observation group The OAA/S score increased,and the amount of esmolol and nitroglycerin decreased(P<0.05).The incidence of adverse reactions in the observation group was lower than that in the control group(P<0.05).Conclusions Propofol combined with remifentanil anesthesia can maintain hemodynamics,improve wake quality,reduce the amount of anesthetic drugs,and reduce the incidence of adverse reactions in elderly patients with hypertension.
