摘要 目的 研究超声造影定量分析评价急性放射性肝损伤(RILD)的应用价值.方法 回顾性分析2014年10月至2019年2月南京医科大学附属无锡第二医院收治的32例成功实施腹部肿瘤三维适形调强放射治疗患者的肝脏作为研究对象,分别于照射前,照射后2周、3周、4周进行超声造影检查,并利用超声造影定量分析软件获取肝实质灌注时间-强度曲线(TIC),脱机分析达峰时间(TTP)、梯度(Grad)、曲线下面积(AUC).结果 与照射前比较,照射后2周、3周、4周组的梯度逐渐降低,TTP逐渐延长(P<0.05);AUC在照射后2周和3周组减低,4周组反而增大(P<0.05).结论 超声造影定量分析参数可作为早期诊断急性放射性肝损伤的重要参考指标. Objective To study the application value of quantitative contrast-enhanced ultrasound in the evaluation of acute radiation-induced liver injury(RILD).Methods A retrospective analysis was performed on the livers of 32 patients who successfully performed three-dimensional conformal intensity modulated radiation therapy for abdominal tumors from October 2014 to February 2019 in Wuxi Second Hospital Affiliated to Nanjing Medical University,before and after irradiation.Contrast-enhanced ultrasound examinations were performed at 2 weeks,3 weeks,and 4 weeks,and TIC curves were obtained using quantitative ultrasound contrast analysis software,and the peak time(TTP),gradient(Grad),and area under the curve(AUC)were analyzed offline.Results Compared with the pre-irradiation,the gradient of the group at 2 weeks,3 weeks,and 4 weeks gradually decreased,and the TTP gradually increased(P<0.05).The AUC decreased at 2 weeks and 3 weeks after the irradiation,but increased at 4 weeks.(P<0.05).Conclusion The quantitative analysis parameters of contrastenhanced ultrasound can be used as an important reference index for early diagnosis of acute radiation-induced liver injury.
