急性脑梗死患者血清TBIL、DBIL、IBIL水平的变化意义

Changes of Serum TBIL DBIL and IBIL Levels in Patients with Acute Cerebral Infarction

目的 探讨急性脑梗死患者血清总胆红素(TBIL)、直接胆红素(DBIL)、间接胆红素(IBIL)水平的变化意义.方法 回顾性分析南京医科大学附属江苏盛泽医院2014年10月至2016年11月收治的急性脑梗死患者247例为病例组,另选取本院同期健康体检者216例为对照组,检测2组TBIL、DBIL和IBIL水平,比较不同梗死部位、不同卒中病因(TOAST)分型及不同梗死体积脑梗死患者血清胆红素水平.结果 与对照组相比,颞叶及基底节梗死组的TBIL、DBIL、IBIL水平均较高;大动脉粥样硬化组的TBIL、IBIL水平均较高;心源性栓塞组的TBIL、DBIL、IBIL水平均较高;大梗死组与中梗死组的TBLI、IBIL、DBIL水平均较高;多发性中小梗死组的TBIL、IBIL水平均较高(P<0.05).结论 血清胆红素水平与脑梗死的部位、病因和病情严重程度密切相关,在临床上通过对血清胆红素水平的检测,可以在一定程度上作为对急性脑梗死病变程度和预后的判断.

Objective To explore the changes of serum TBIL,DBIL and IBIL levels in patients with acute cerebral infarction.Methods A retrospective analysis was made on 247 patients with acute cerebral infarction who were admitted to Jiangsu Shengze Hospital Affiliated to Nanjing Medical University,from October 2014 to November 2016.Another 216 healthy people in our hospital at the same time were selected as the control group.The serum total bilirubin(TBIL),direct bilirubin(DBIL)and indirect bilirubin(IBIL)levels of the two groups were measured,and the serum levels of different infarction sites,different TOAST types and different infarction volumes of cerebral infarction patients were compared Bilirubin level.Results Compared with the control group,TBIL,DBIL and IBIL in the infarction group of temporal lobe and basal ganglia were higher;TBIL and IBIL in the large atherosclerosis group were higher;TBIL,DBIL and IBIL in the cardiogenic embolism group were higher;TBIL,IBIL and DBIL in the large infarction group and the middle infarction group were higher;TBIL and IBIL in the multiple small and medium infarction group were higher(P<0.05).Conclusion The level of serum bilirubin is closely related to the location,etiology and severity of cerebral infarction.The detection of serum bilirubin can be used to judge the degree of pathological changes and prognosis of acute cerebral infarction to a certain extent.
