关节镜对膝关节腘窝囊肿患者疼痛及IL-6、CRP水平的影响

Effects of Arthroscopy on Pain and IL-6 and CRP Levels in Patients with Popliteal Cysts of the Knee Joint

目的 探究关节镜对膝关节腘窝囊肿患者疼痛及血清白介素-6(IL-6)、C反应蛋白(CRP)水平的影响.方法 回顾性分析如皋市中医院2017年4月至2019年4月接受治疗的76例膝关节腘窝囊肿患者.按照治疗方式的不同将患者分为对照组与实验组,各38例.对照组患者采取开放式手术进行治疗,实验组患者则给予关节镜下囊肿内引流术进行治疗,比较2组治疗后3个月临床有效率、手术情况及并发症发生情况,比较2组术前及术后7d内疼痛程度变化,比较2组患者术前及术后3d炎性因子水平.结果 治疗后3个月实验组临床有效率94.74%显著高于对照组68.42%(P<0.05),实验组手术时间、切口长度、术中出血量及住院时间均显著短于或少于对照组(P<0.05),术后1~7d随时间的延长2组视觉模拟疼痛(VAS)评分呈下降趋势,术后3d及术后7d实验组显著低于对照组(P<0.05),与术前相比,术后3d 2组血清IL-6、CRP水平降低,实验组低于对照组(P<0.05).实验组并发症发生率15.79%显著低于对照组36.84%(P<0.05).结论 关节镜下囊肿内引流术治疗膝关节腘窝囊肿可明显提高临床疗效,降低炎症因子水平,减轻疼痛,安全性较高,促进患者康复,值得临床推广应用.

Objective To investigate the effect of arthroscopy on pain and IL-6,CRP levels in patients with popliteal cysts of the knee joint.Methods A retrospective analysis of 76 patients with popliteal cysts of the knee joint treated in Rugao Traditional Chinese Medicine Hospital from April 2017 to April 2019.According to the different treatment methods,patients were divided into control group and experimental group,38 cases each.Patients in the control group were treated with open surgery,and patients in the experimental group were treated with arthroscopic intracystic drainage.The clinical efficiency,surgical status,and complications of the two groups were compared 3 months after treatment,and the preoperative group was compared.The pain levels were changed within 7 days after operation,and the levels of inflammatory factors were compared between the two groups before and after 3 days.Results Three months after treatment,the clinical effective rate of the experimental group was 94.74%,which was significantly higher than that of the control group 68.42%(P<0.05),and the operation time,incision length,intraoperative bleeding volume and hospitalization time of the experimental group were significantly shorter than or less than that of the control group(P<0.05),the VAS scores of the two groups showed a downward trend with the extension of time from 1 to 7 days after surgery.The experimental group was significantly lower than the control group at 3 days and 7 days after surgery(P<0.05).Compared with that before surgery,3 days after surgery The levels of serum IL-6 and CRP in the two groups decreased,and the experimental group was lower than the control group(P<0.05).The complication rate in the experimental group was 15.79%,which was significantly lower than that in the control group,36.84%(P<0.05).Conclusion Treatment of knee joint popliteal cysts with arthroscopic intracystic drainage can significantly improve clinical efficacy,reduce inflammatory factor levels,reduce pain,have higher safety,promote patient recovery,and is worthy of clinical promotion and application.
