PG水平检测用于评估胃癌风险的临床价值

To Explore the Clinical Value of PG Level Detection for Assessing the Risk of Gastric Cancer

目的 探析对胃癌患者进行血清胃蛋白酶原(PG)水平检测对于病情判断及引导的临床价值.方法 回顾性分析2018年10月至2019年9月西南医科大学附属中医医院接受诊治的患者群体,在经手术切除或胃镜活检,病理诊断确诊为非萎缩性胃炎、胃溃疡、萎缩性胃炎以及胃癌患者中各随机抽取30例,分别设为A组、B组、C组及D组,所有患者均进行血清PGⅠ与PGⅡ检测,比较各组间PGⅠ、PGⅡ、PGⅠ/PGⅡ(PGR)水平及阳性率,比较PGⅠ、PGⅡ、PGⅠ/PGⅡ(PGR)诊断价值.结果 相对于D组患者,A、B、C组接受血清检测提示其PGⅠ、PGR指标更高(P<0.05),B组患者血清PGⅡ指标高于A、B、C组患者群体且差异有统计学意义(P<0.05),PGⅠ阳性率,A组最低,B组最高,C、D组相同,PGⅡ阳性率,B、C组低于A、D组,PGR阳性率A、B、C组低于D组,4组间PGⅠ、PGⅡ及PGR阳性率经比较差异无统计学意义(P>0.05);单项诊断指标结果 对比,PGⅠ敏感度、准确度明显优于PGⅡ、PGR,PGR特异度明显优于PGⅠ+PGⅡ,PGⅠ+PGⅡ+PGR联合诊断敏感度、特异度及准确度均显著高于各项单独诊断(P<0.05).结论 对于胃癌前病变以及胃癌患者进行血清PG水平检测能够有良好的临床指导价值,可作为一项血清学诊断依据.

Objective To explore the clinical value of PG level detection for the diagnosis and guidance of gastric cancer patients.Methods Were retrospectively analyzed in October 2018 to September 2019,southwest medical university affiliated hospital of traditional Chinese medicine diagnosis and treatment of patients'groups,in the surgical resection or gastroscope biopsy and pathology diagnosis of atrophic gastritis,gastric ulcer and atrophic gastritis and gastric cancer patients randomly selected 30 cases,respectively,set as group A and group B,group C and group D,all patients with serum PGⅠwith PGⅡdetection,comparison between groups of PGⅠ,PGⅡ,PGⅠ/PGⅡ(PGR)levels and the positive rate,comparing PGⅠ,PGⅡ,PGⅠ/PGⅡ(PGR)diagnostic value.Results Relative to patients in group D,A,B and C group prompt its tested for serum PGⅠ,PGR index was significantly higher(P<0.05),group B patients serum PGⅡindex is significantly higher than group A,B,C groups with statistical significance(P<0.05),PGⅠpositive rate,minimum group A,group B,C,D group,same PGⅡpositive rate,lower than group A,D,B and C group PGR positive rate of group A,B,C lower than that of group D,four groups between PGⅠ,PGⅡand PGR positive comparative difference was not significant(P>0.05);The sensitivity and accuracy of PGI were better than that of PGI,PGR and PGR.PGⅠ+PGⅡ+PGR joint diagnosis accuracy and sensitivity,specific degrees were significantly higher than the single diagnosis(P<0.05).Conclusion The detection of serum PG levels in patients with gastric precancerous lesions and gastric cancer has good clinical guidance value and can be used as a serological diagnosis basis.
