唾液微小RNA-214在心力衰竭诊断的临床价值 被引量:2

摘要 目的探讨唾液微小RNA(microRNA,miR)-214在心力衰竭诊断的可行性和临床价值。方法入选我院临床诊断心力衰竭患者51例(心衰组),对照组采用2?1的方式选取同期来我院体检者24例。收集患者晨起端坐位10 ml唾液,提取RNA,定量RT-PCR检测6种唾液microRNA(miR-1,hmiR-18b-3p,miR-199a-3p,miR-214,miR-211,let-7b)。结果6种唾液microRNA仅miR-214在心衰组显著高于对照组(2.80±0.34 vs 1.57±0.30,P=0.027);ROC曲线分析显示,miR-214在心力衰竭诊断中曲线下面积(AUC)=0.694(95%CI:0.513~0.778,P=0.007),N末端B型钠尿肽前体(NT-proBNP)AUC=0.924(95%CI:0.860~0.988,P=0.000);miR-214与NT-proBNP对比显示,P=0.002,miR-214的临床诊断价值低于NT-proBNP。结论唾液microRNA可用于心力衰竭的诊断,miR-214具有一定的诊断意义,需要进一步探讨其他可能的microRNA作为生物标志物的可行性。 Objective To study the clinical value of salivary miR-214 in diagnosis of heart failure(HF).Methods Fifty-one HF patients in our hospital served as a HF group.The control group selected 24 HF-free patients who came to our hospital for physical examination in the same period by two to one.Ten ml saliva was taken from the HF patients in morning at a sitting position to extract RNA.Six kinds of salivary microRNA(miR-1,miR-18 b-3 p,miR-199 a-3 p,miR-214,miR-211,let-7 b)were detected by quantitative reverse transcription-polymerase chain reaction.Results The miR-214 level was significantly higher in HF group than in control group(2.80±0.34 vs 1.57±0.30,P=0.027).ROC curve analysis showed that the AUC for miR-214 and NT-proBNP in diagnosis of HF was 0.694 and 0.924 respectively(95%CI:0.513-0.778,P=0.007;95%CI:0.860-0.988,P=0.000).The value of miR-214 was lower than that of NT-proBNP in diagnosis of HF(χ~2=9.52,P=0.002).Conclusion Salivary microRNA can be used in diagnosis of HF and miR-214 plays a certain role in diagnosis of HF.The feasibility to use salivary microRNA as a biomarker of HF needs to be further studied.
