阴茎恶性淋巴瘤1例并文献复习

Malignant lymphoma of penis:a case report and review of the literature

目的探究阴茎恶性淋巴瘤的诊断方法及最佳治疗方案,以提高其诊治水平。方法通过回顾我院1例阴茎恶性淋巴瘤患者的临床资料及诊治经过,并结合相关文献进行分析、讨论及总结。结果患者行阴茎结节活检术,术后病检及免疫组化确诊为阴茎恶性淋巴瘤(非霍奇金弥漫大B细胞淋巴瘤),予以R-CVP方案(利妥昔单抗+环磷酰胺+长春新碱+醋酸泼尼松)完成第一周期化疗后,阴茎头肿物缩小,阴茎肿胀明显减轻。结论阴茎恶性淋巴瘤症状无特异性,诊断困难,术前活检病理及免疫组化如能确诊可避免手术,早期行全身规律化疗效果较好,但目前仍缺乏标准治疗方案。

Objective To explore the diagnostic methods and best treatment for penil malignant lymphoma so as to improve the level of diagnosis and treatment for this disease.Methods By reviewing the clinical data,diagnosis and treatment process from a patient in our hospital,we analysed,discussed and summarised the characteristics of penile malignant lymphoma combined with relevant literature.Results The patient underwent penile nodule biopsy.The postoperative exsmination and immunohistochemistry were diagnosed as penile malignant lymphoma(non-Hodgkin diffuse large B-cell lymphoma).R-CVP regimen(rituximab+cyclophosphamide+vincristine+prednisone acetate)was recommended for chemotherapy.After the first cycle of chemotherapy,the balanus mass and penile swelling significantly relieved.Conclusions The symptoms of penile malignant lymphoms are not specific,so this disease is difficult to be diagnosed.In case of the preoperative pathological biopsy and immunohistochemistry can confirm the lymphoma,the surgery can beavoided.Early systemic chemotherspy is satisfied.However,there is still no standard treatment.
