高压氧联合虚拟现实技术对重型颅脑损伤患者认知运动功能康复的疗效研究 被引量:9

Effect of hyperbaric oxygen combined with virtual reality technology on cognitive motor function rehabilitation in patients with severe craniocerebral injury

摘要 目的探索高压氧联合虚拟现实技术对重型颅脑损伤患者认知和运动功能的康复疗效。方法共选取93例重型颅脑损伤患者将其随机分为高压氧组31例、虚拟现实技术组31例及综合治疗组31例。3组患者均给予常规外科手术处理病情稳定神志清醒,高压氧组在此基础上辅以高压氧治疗,虚拟现实技术组则辅以虚拟现实技术训练,综合治疗组则辅以高压氧、虚拟现实技术训练和运动作业治疗。于治疗前、治疗4周后分别采用中文版蒙特利尔认知评估量表(MoCA)、改良Barthel指数(MBI)以及Fugl-Meyer评分(FMA)进行评定。结果治疗前,3组患者格拉斯哥昏迷量表(GCS)、MoCA、MBI、FMA评分差异无统计学意义(P>0.05)。治疗4周后,3组患者的MoCA、MBI、FMA的评分均较治疗前明显提高(P<0.05),高压氧组均较虚拟现实技术组MoCA评分有明显提高(P<0.05),且虚拟现实技术组均较高压氧组MBI、FMA评分明显提高(P<0.05),综合治疗组较高压氧组和虚拟现实技术组的MoCA、MBI、FMA的评分有明显提高(P<0.05)。结论高压氧联合虚拟现实技术对重型颅脑损伤患者的认知和运动功能康复疗效显著,值得推广应用。 Objective To observe the effect of hyperbaric oxygen combined with virtual reality on cognitive and motor function of patients with severe craniocerebral injury.Methods A total of 93 patients with severe craniocerebral injury were randomly divided into hyperbaric oxygen group(31 cases),virtual reality group(31 cases)and combined therapy group(31 cases).Patients in the 3 groups were given conventional surgery to treat patients with stable and conscious state.The hyperbaric oxygen group was supplemented with hyperbaric medicine,while the virtual reality group was supplemented with virtual reality training.The combined therapy group was supplemented with hyperbaric oxygen,virtual reality training and exercise therapy.The Montreal cognitive assessment(MoCA)scale,modified barthel index(MBI)and Fugl-Meyer assessment(FMA)were used before treatment and 4 weeks after treatment.Results Before treatment,there was no significant difference in Glasgow coma scale(GCS),MoCA,MBI and FMA scores between the three groups(P>0.05).Four weeks after treatment,3 groups of patients with MoCA,MBI,FMA score was improved obviously the group(P<0.05),all MoCA scores in the hyperbaric oxygen were higher than those in the virtual reality technology group(P<0.05).MBI and FMA scores in the virtual reality group were significantly higher than those in the hyperbaric oxygen group(P<0.05).The MoCA,MBI,FMA of comprehensive treatment group have improved obviously than the virtual reality technology and the hyperbaric oxygen group(P<0.05).Conclusion The effect of hyperbaric oxygen combined with virtual reality on the rehabilitation of cognitive and motor function of patients with severe craniocerebral injury is improved obviously,which is worthy of promotion and application.
