研究内口挂线引流联合开放式瘘管切除术治疗肛瘘的效果

To Study the Effect of Internal Thread Drawing Drainage combined with Open Fistulectomy in the Treatment of Anal Fistula

目的研究内口挂线引流联合开放式瘘管切除术治疗肛瘘的效果。方法观察2016年10月至2018年12月时间段内医院肛肠科符合临床要求的40例肛痿患者,根据随机数表法分组,即对照组与观察组,各20例。对照组经常规手术治疗,观察组经内口挂线引流联合开放式瘘管切除术治疗。比较两组手术疗效、并发症发生率及切口康复情况。结果观察组临床总有疗效为90.00%,对照组临床总有疗效为60.00%,观察组高于对照组,差异有统计学意义(P<0.05)。观察组肛门失禁发生率、切口感染率及痿管增加率、贫血发生率均为5.00%,对照组相对应为35.00%、35.00%、40.00%、40.00%,观察组均低于对照组,差异有统计学意义(P<0.05)。观察组创口出血评分、伤口愈合时间、创缘水肿时间及住院时间分别为(2.22±0.35)分、(20.32±2.21)d、(1.61±0.17)d及(20.32±2.12)d,对照组依次为(3.69±0.56)分、(24.66±2.32)d、(2.76±0.69)d及(23.27±2.32)d,观察组均低于对照组,差异有统计学意义(P<0.05)。结论经内口挂线引流联合开放式瘘管切除术治疗肛瘘,效果显著,有利于促进患者术后康复,具有较高的应用及推广价值。

Objective To study the effect of internal thread drawing drainage combined with open fi stulectomy in the treatment of anal fi stula.Methods 40 patients with anal fl accidity who met the clinical requirements in the anorectal Department of the hospital during the period from October 2016 to December 2018 were observed.They were divided into control group and observation group according to the random number table method,20 in each group.The control group was treated by routine operation,and the observation group was treated by internal thread drawing and open fi stulectomy.The operative effect,complication rate and incision rehabilitation were compared between the two groups.Results The total clinical effi cacy of the observation group was 90.00%,the total clinical effi cacy of the control group was 60.00%,the difference was statistically signifi cant(P<0.05).The incidence of anal incontinence,incision infection,impotence and anemia were all 5.00%in the observation group,and 35.00%,35.00%,40.00%and 40.00%in the control group.The difference was statistically signifi cant(P<0.05).The bleeding score,wound healing time,edema time and hospitalization time of the observation group were(2.22±0.35),(20.32±2.21)d,(1.61±0.17)d and(20.32±2.12)d,respectively,and those of the control group were(3.69±0.56),(24.66±2.32)d,(2.76±0.69)d and(23.27±2.32)d,respectively.The difference was statistically signifi cant(P<0.05).Conclusion The treatment of anal fi stula by internal thread drainage combined with open fi stulectomy has a signifi cant effect,which is conducive to promoting the postoperative rehabilitation of patients,and has a high application and promotion value.
