食管癌手术病人出院后支持性照护需求量表的编制及信效度检验

Development and reliability and validity test of Supportive Care Needs Scale for Esophagectomy Patients after Discharge(SCNS ED)

[目的]构建食管癌手术病人出院后支持性照护需求量表,并进行信效度检验。[方法]以支持性照护需求理论框架为基础,通过文献回顾、质性访谈、德尔菲专家咨询法形成量表的临床测试版本。选取江苏省3所三级甲等医院食管癌术后出院病人319例进行调查,测定量表的信效度。[结果]最终形成的食管癌手术病人出院后支持性照护需求量表包括生理需求、信息需求、情感/心理需求、社会需求4个维度,共49个条目。量表的内容效度指数为0.97,各条目的内容效度指数为0.78~1.00。探索性因子分析提取了4个公因子,累积方差贡献率为61.587%。总量表的Cronbach′sα系数为0.971,各维度的Cronbach′sα系数为0.894~0.946,量表的折半信度系数为0.935。量表总分与欧洲癌症病人生命质量核心量表(EORTC QLQ-C30)、欧洲癌症病人生命质量-食管癌专用量表(EORTC QLQ-OES18)总分及各维度得分显著相关,与癌症病人支持性照护需求量表(SCNS-SF34)总分及各维度呈正相关(P<0.01)。[结论]形成的食管癌手术病人出院后支持性照护需求量表的信效度良好,可用于食管癌手术病人出院后支持性照护需求的评估。

Objective:To develop the Supportive Care Needs Scale For Esophagectomy Patients After Discharg,and to test its reliability and validity.Methods:Under the theoretical framework of supportive care needs,literature review,qualitative interview,and Delphi method were used to verify the clinical test version of scale.A total of 319 esophageal cancer patients discharged from 3 tertiary grade A hospitals in Jiangsu province,were selected as subjects.And the reliability and validity of scale were tested.Results:The final Supportive Care Needs Scale for Esophagectomy Patients After Discharg included following 4 dimensions:physiological needs,information needs,emotional or psychological needs,and social needs,involving 49 items.The content validity index was 0.97,and the content validity indexes of each items ranged from 0.78 to 1.00.Exploratory factor analysis(EFA)extracted 4 factors,whose cumulative contribution of variance accounted to 61.587%.The Cronbach′sαcoefficients of the total scale was 0.971.And the Cronbach′sαcoefficients of each dimension were 0.894~0.946.The split reliability coefficient of the scale was 0.935.The total score of developed scale was significantly correlated with total scores of EORTC QLQ C30 and EORTC QLQ OES18 and scores of each dimension.And it was positively correlated with total score of SCNS SF34 and scores of each dimension(P<0.01).Conclusion:Supportive Care Needs Scale for Esophagectomy Patients After Discharge has good reliability and validity.The scale can be used as a tool to evaluate supportive care needs of esophagectomy patients after discharge.
