1125例多囊卵巢综合征患者中医体质类型分布及演变规律

Distribution and Evolution of TCM Constitutions of 1125 Patients with Polycystic Ovary Syndrome

目的研究多囊卵巢综合征中医体质分布情况及兼夹体质演变规律。方法运用EXCEL表格、SPSS 20. 0统计软件对1125例PCOS患者体质转化分进行分析。结果 (1) PCOS患者"基本平和质"以倾向气虚质及倾向湿热质为多见;(2) PCOS患者体质类型为单一偏颇体质占20%,以单一气虚质为主,其次为单一血瘀质;其倾向体质以倾向血瘀质、湿热质、阴虚质为主;(3)在605例兼夹体质中,具有两种兼夹体质的患者约24. 96%(151/605),具有3种及以上兼夹体质的患者约75. 04%(454/605);当"基本体质"为血瘀质时,主要兼夹体质为"血瘀-气虚质";当"基本体质"为气虚质时,"气虚-血瘀质"为主要兼夹体质;当"基本体质"为湿热质时,"湿热-血瘀质"为主要兼夹体质。结论 PCOS患者体质类型以兼夹体质多见,兼夹体质间相互联系,相互影响,相互促进,血瘀质、气虚质及湿热质是本病的核心体质类型。临床中应充分辨明兼夹体质类型,从而提高诊治本病的疗效。

Objective To study the distribution of TCM constitutions and the discipline of constitution evolution in polycystic ovary syndrome.Methods The body transformation scores of 1125 patients with PCOS were analyzed by EXCEL table and SPSS 20.0 statistical software.Results(1)PCOS patients with“tendency of special constitution”were mostly inclined to Qi deficiency constitution and inclined to damp-heat constitution.(2)Among the PCOS patients,the single biased constitution accounted for 20%,which was dominated by single Qi deficiencyconstitution,followed by single blood stasis constitution.These TCM constitutional tendencies tended to be dominated by blood stasis,damp-heatand Yin deficiencyconstitution.(3)Among 605 cases of composite constitutions,about 24.96%(151/605)of patients had two types of composite constitutions,and about 75.04%(454/605)of patients with three or more composite constitutions.When the“basic constitution”was blood stasisconstitution,the main composite constitutionswas“blood stasis-Qi deficiencyconstitution”.When the“basic constitution”was Qi deficiencyconstitution,“Qi deficiency-blood stasis constitution”was the main composite constitutions.When the“basic constitution”was damp-heatconstitution,“damp-heat-blood stasis constitution”was the main composite constitutions.Conclusion The constitution of PCOS patients is more common with composite constitutions,which are interrelated,mutually influentialand mutually reinforcing,blood stasis,Qi deficiency and damp-heat constitutions are the core constitution types of this disease.In clinical practice,the types of composite constitutions should be fully discriminatedso as to improve the efficacy of diagnosis and treatment of this disease.
