负压封闭引流和游离皮瓣移植对慢性溃疡创面愈合的影响

Eeffect of vacuum sealing drainage combined with free skin flap transplantation on wound healing of chronic ulcer

目的观察负压封闭引流(VSD)联合游离皮瓣移植对促进慢性溃疡创面愈合的临床效果。方法将我院收治的82例慢性皮肤溃疡创面患者随机分为对照组和观察组,各41例。在清创治疗基础上,对照组应用游离皮瓣移植,观察组给予VSD+游离皮瓣移植治疗。比较两组创面愈合效果。结果观察组创面愈合时间、换药次数、住院时间均优于对照组(P<0.05)。术后,两组HB、VEGF、TcPO2水平均升高,NRS、瘢痕评分均降低,且观察组优于对照组(P<0.05)。结论采用VSD+游离皮瓣移植术能有效促进肉芽组织的微血管管再生,促进创面愈合,减轻疼痛及瘢痕生长。

Objective To observe the clinical effect of vacuum sealing drainage(VSD)combined with free skin flap transplantation in promoting wound healing of chronic ulcer.Methods Eighty-two patients with chronic skin ulcer wounds admitted in our hospital were randomly divided into control group and observation group,with 41 cases in each group.On the basis of debridement treatment,the control group was treated with free skin flap transplantation,while the observation group was treated with VSD+free skin flap transplantation.The wound healing effects of the two groups were compared.Results The wound healing time,dressing change times and hospitalization time in the observation group were better than those in the control group(P<0.05).After operation,the levels of Hb,VEGF and TcPO2 in the two groups increased,the scores of NRS and scar decreased,and those of the observation group were better than the control group(P<0.05).Conclusion VSD+free skin flap transplantation can effectively promote microvascular regeneration of granulation tissue,promote wound healing,and relieve pain and scar growth.
