3.0T磁共振TIC曲线分型在不典型骨转移瘤与感染类骨疾病鉴别诊断中的应用价值

摘要 目的分析3.0 T磁共振时间-信号强度曲线(TIC曲线)分型在不典型骨转移瘤与感染类骨疾病鉴别诊断中的应用价值。方法收集2016年1月至2019年1月于我院确诊的90例不典型骨转移瘤及感染类骨疾病患者的影像资料,回顾性分析患者TIC曲线图像资料,评价TIC曲线分型对不典型骨转移瘤与感染类骨疾病的鉴别诊断价值。结果不典型骨转移瘤组的39例患者中TIC-Ⅰ型1例(2.56%),TIC-Ⅱ型16例(41.03%),TIC-Ⅲ型22例(56.41%);感染类骨疾病组的51例患者中TIC-Ⅰ型35例(68.63%),TIC-Ⅱ型15例(29.41%),TIC-Ⅲ型1例(1.96%)。不典型骨转移瘤组与感染类骨疾病组患者的TIC曲线分型比较,差异具有统计学意义(P<0.05)。与病理结果比较,TIC诊断骨骼系统良恶性病灶的灵敏度为92.31%,特异度为90.20%。结论TIC曲线不同类型在不典型骨转移瘤与感染类骨疾病的鉴别诊断中有很高的应用临床价值。 Objective To analyze the application value of 3.0 T magnetic resonance time-signal intensity curve(TIC curve)in the differential diagnosis of atypical bone metastases and infectious bone diseases.Methods The image data of 90 patients with atypical bone metastases and infectious bone diseases diagnosed in our hospital from January 2016 to January 2019 were collected,and the TIC curve image data of patients were retrospectively analyzed.The differential diagnosis value of TIC curve typing for atypical bone metastases and infectious bone diseases was evaluated.Results Among the 39 patients in the atypical bone metastasis group,1 case was TIC-Ⅰ(2.56%),16 cases were TIC-Ⅱ(41.03%),and 22 cases were TIC-Ⅲ(56.41%);among the 51 patients in the infectious bone disease group,35 cases were TIC-Ⅰ(68.63%),15 cases were TIC-Ⅱ(29.41%),and 1 case was TIC-Ⅲ(1.96%).The difference in TIC curve typing between the patients in the atypical bone metastases group and the patients in the infectious bone disease group was statistically significant(P<0.05).Compared with pathological results,the sensitivity of TIC to diagnose benign and malignant lesions of the skeletal system was 92.31%and the specificity was 90.20%.Conclusion Different types of TIC curves have high clinical value in the differential diagnosis of atypical bone metastases and infectious bone diseases.
