摘要
目的探讨血管介入治疗难治性产后出血的效果及对凝血因子的影响。方法回顾性分析2016年5月至2017年12月我院收治的130例难治性产后出血患者的临床资料,依据不同治疗方式将患者分为对照组和观察组,各65例。对照组应用双侧子宫动脉结扎术,研究组应用经导管动脉栓塞术。比较两组的治疗效果。结果研究组产后2、24 h的出血量均显著低于对照组(P<0.05)。治疗后,两组的凝血因子Ⅷ、凝血因子Ⅸ及ANG-Ⅱ水平均升高,且研究组高于对照组(P<0.05)。研究组的恶露持续时间、月经复潮时间及月经周期均显著短于对照组(P<0.05)。结论血管介入治疗应用于难治性产后出血患者中可显著降低患者的产后出血量,提高相关凝血因子水平,缩短恶露持续时间、月经复潮时间及月经周期。
Objective To investigate the effect of vascular interventional therapy on refractory postpartum hemorrhage and its influence on coagulation factors.Methods The clinical data of 130 patients with refractory postpartum hemorrhage treated in our hospital from May 2016 to December 2017 were retrospectively analyzed.The patients were divided into control group and study group according to the different treatment methods,with 65 cases in each group.The control group was treated with bilateral uterine artery ligation,and the study group was treated with transcatheter arterial embolization.The treatment effects of the two groups were compared.Results The amount of bleeding at 2 and 24 hours postpartum in the study group were significantly lower than those in the control group(P<0.05).After treatment,the levels of coagulation factorⅧ,coagulation factorⅨand ANG-Ⅱin the two groups increased,and those in the study group were higher than the control group(P<0.05).The duration of lochia,menstrual rejuvenation time and menstrual cycle in the study group were significantly shorter than those in the control group(P<0.05).Conclusion Vascular interventional therapy applied to patients with refractory postpartum hemorrhage can significantly reduce the amount of postpartum hemorrhage,increase the level of related coagulation factors,shorten the duration of lochia,menstrual rejuvenation time and menstrual cycle.
