摘要
目的探讨重症肺炎患者机械通气期间气道分级管理的效果。方法选取2018年3月至2018年12月我院收治的重症肺炎患者145例,随机将其分为对照组(72例)和研究组(73例)。对照组进行常规机械通气治疗,研究组在机械通气的基础上采用气道分级管理治疗。比较两组患者的机械通气时间、住院时间、并发症发生情况及机械通气相关指标。结果研究组患者的机械通气时间和住院时间均短于对照组,差异具有统计学意义(P<0.05)。研究组患者的脑缺氧、肾脏功能损伤及深静脉血栓的发生率均显著低于对照组,差异具有统计学意义(P<0.05)。研究组的机械通气指标均优于对照组,差异具有统计学意义(P<0.05)。结论在重症肺炎患者机械通气期间应用气道分级管理能促进患者痰液引流和病情恢复,并且在低频次的物理治疗中既能保证机械通气效果,又能减少胸部物理治疗的相关副作用,且能明显提高重症肺炎患者的机械通气指标,值得临床推广应用。
Objective To investigate the effect of airway grading management during mechanical ventilation in patients with severe pneumonia.Methods A total of 145 patients with severe pneumonia admitted in our hospital from March 2018 to December 2018 were selected and randomly divided into control group(72 cases)and research group(73 cases).The control group was treated with conventional mechanical ventilation,and the research group was treated with airway grading management on the basis of mechanical ventilation.The time of mechanical ventilation and hospitalization,complications and related indexes of mechanical ventilation of the two groups were compared.Results The time of mechanical ventilation and hospitalization in the research group were shorter than those in the control group,the differences were statistically significant(P<0.05).The incidences of cerebral hypoxia,renal dysfunction and deep vein thrombosis in the research group were significantly lower than those in the control group,the differences were statistically significant(P<0.05).The mechanical ventilation indexes in the research group were better than those in the control group,the differences were statistically significant(P<0.05).Conclusion During the mechanical ventilation in patients with severe pneumonia,the application of airway grading management can promote the drainage of sputum and the recovery of disease,and it can not only ensure the effect of mechanical ventilation in the low-frequency physical therapy,but also reduce the related side effects of chest physical therapy,and can significantly improve the mechanical ventilation indexes of patients with severe pneumonia,which is worthy of clinical promotion and application.
