抗中性粒细胞胞浆抗体阳性患者相关实验指标水平及与抗核抗体的关系

Relationship between the level of related experimental indexes and antinuclear antibodies in patients with positive antineutrophil cytoplasmic antibody

目的探讨抗中性粒细胞胞浆抗体(ANCA)阳性患者相关实验指标水平及与抗核抗体(ANA)的关系。方法选取2017年1月至2019年5月咸阳市中心医院收治的40例ANCA阳性患者为试验组,并纳入同时期40例健康体检者为对照组。检测两组受检者Crea、BUN、UA、Cys-C、D-D、FIB、FDP、ANCA和ANA水平;检测两组MPO-Ab、PR3-Ab、ENA抗体。比较两组受试者血清中ANA、抗ENA抗体阳性率及上述各实验指标水平。结果试验组的Crea、BUN、UA、Cys-C、D-D、FDP水平明显高于对照组(P<0.05)。试验组的ANA阳性率明显高于对照组(P<0.05)。可以检测到MPO或PR3靶抗原抗体的p-ANCA阳性患者血清中Crea、BUN水平明显高于未检测到MPO或PR3靶抗原抗体的p-ANCA阳性患者(P<0.05)。结论ANCA阳性患者会出现肾功能损害及凝血过程激活趋势,故ANCA检测对继发性血管炎引起的相关疾病的诊断与治疗具有重要的指导意义。

Objective To explore the relationship between the level of related experimental indexes and antinuclear antibodies(ANA)in patients with positive antineutrophil cytoplasmic antibody(ANCA).Methods A total of 40 ANCA positive patients in Xianyang central hospital from January 2017 to May 2019 were selected as experimental group,and 40 healthy people in the same period were included as control group.The levels of Crea,BUN,UA,Cys-C,D-D,FIB,FDP,ANCA and ANA were measured in the two groups;the MPO-Ab,PR3-Ab and ENA antibodies were measured in the two groups.The positive rates of ANA,anti-ENA antibodies and the above experimental indicators were compared between the two groups.Results The levels of Crea,BUN,UA,Cys-C,D-D and FDP in the experimental group were significantly higher than those in the control group(P<0.05).The positive rate of ANA in the experimental group was significantly higher than that in the control group(P<0.05).The levels of serum Crea and BUN in p-ANCA positive patients with MPO or PR3 target antigen antibody detected were significantly higher than those in p-ANCA positive patients without MPO or PR3 target antigen antibody detected(P<0.05).Conclusion In ANCA positive patients have renal function damage and activation trend of coagulation process,so ANCA detection has important guiding significance for the diagnosis and treatment of related diseases caused by secondary vasculitis.
