超声引导下颈神经根脉冲射频联合小针刀对神经根型颈椎病患者疼痛程度及颈椎关节活动度的影响

Effect of ultrasound-guided cervical nerve root pulse radiofrequency combined with small needle knife on pain degree and cervical vertebra joint mobility in patients with cervical spondylotic radiculopathy

目的探讨超声引导下颈神经根脉冲射频联合小针刀对神经根型颈椎病(CSR)患者疼痛程度及颈椎关节活动度的影响。方法将2017年2月至2019年1月我院收治的100例CSR患者采用简单随机化分组法分为联合组和小针刀组,各50例。小针刀组予以小针刀治疗,联合组予以超声引导下颈神经根脉冲射频联合小针刀治疗。比较两组的治疗效果。结果联合组的治疗总有效率高于小针刀组(P<0.05)。治疗后3、7、14 d,两组VAS评分均低于治疗前,且联合组低于小针刀组(P<0.05)。与治疗前相比,两组治疗后14 d颈椎曲度、颈椎屈伸活动度均升高(P<0.05),但组间比较差异无统计学意义(P>0.05)。结论超声引导下颈神经根脉冲射频联合小针刀治疗CSR可实现精准定位与穿刺,减轻患者的疼痛程度,提高患者的颈椎功能,安全可靠。

Objective To investigate the effect of ultrasound-guided cervical nerve root pulse radiofrequency acupuncture combined with small needle knife on the degree of pain and cervical vertebra joint mobility in patients with cervical spondylotic radiculopathy(CSR).Methods A total of 100 patients with CSR enrolled in our hospital from February 2017 to January 2019 were divided into combined group and small needle knife group by simple randomization method,with 50 cases in each group.The small needle knife group was treated with small needle knife.The combined group was treated with ultrasound-guided cervical nerve root pulse radiofrequency combined with small needle knife.The therapeutic effects of the two groups were compared.Results The total effective rate of treatment in the combined group was higher than that in the small needle knife group(P<0.05).At the 3rd,7th and 14th days after treatment,the VAS scores of the two groups were lower than those before treatment,and those of the combined group were lower than the small needle knife group(P<0.05).At 14th day after treatment,the cervical curvature and flexion and extension activity of the two groups increased compared with those before treatment(P<0.05),but there were no significant differences between the two groups(P>0.05).Conclusion Ultrasound-guided cervical nerve root pulse radiofrequency combined with small needle knife for CSR can achieve precise positioning and puncture,reduce the degree of pain and improve the cervical function of patients,which is safe and reliable.
