摘要
目的探究膝关节后交叉韧带(PCL)内侧滑膜皱襞的磁共振成像(MRI)表现,并评估不同滑膜皱襞类型与PCL撞击之间的关系。方法选取68例接受膝关节MRI和关节镜检查的患者,由2名放射科医师进行回顾性评估。MRI将内侧滑膜皱襞分3类:A型显示无内侧滑膜皱襞;B型显示小的滑膜皱襞但未超出假象线;C型显示滑膜皱壁超出假想线。关节镜记录PCL撞击的发生。结果观察者内对内侧滑膜皱襞分类具有高度一致性(κ=0.830)。MRI和关节镜具有高度一致性(Kappa=0.82)。28例为A型,21例为B型,19例为C型。在关节镜检查中发现了28例PCL撞击,C型发生撞击的可能性明显高于A型。结论在3种类型的内侧滑膜皱襞中,C型与关节镜下PCL的撞击最相关。因此,C型皱襞可能是膝关节内紊乱的原因之一。
Objective To investigate the magnetic resonance imaging(MRI)features of medial synovial plica of posterior cruciate ligament(PCL)of knee joint,and to evaluate the relationship between different types of synovial plica and PCL impact.Methods A total of 68 patients were selected for MRI and arthroscopy.A retrospective assessment was performed by 2 radiologists.MRI classified the medial synovial plica into three categories:type A showed no medial synovial plica;type B showed small synovial plica but not beyond the imaginary line;and type C showed plica beyond the imaginary line.Impact of the PCL were obtained from arthroscopic records.Results The observers were highly consistent with the classification of medial synovial plica(κ=0.830).MRI and arthroscopy were highly consistent(Kappa=0.82).A total of 28 cases were type A,21 cases were type B,and 19 cases were type C.Twenty-eight cases of PCL impacts were found in arthroscopy,and the impact likelihood of type C was significantly higher than that of type A.Conclusion Among the three types of medial synovial plica,type C is most relevant to the impact of arthroscopic PCL.Therefore,C-type plica may be the cause of internal turbulence in the knee.
