NLR对冠心病及病变严重程度诊断价值分析 被引量:1

Analysis on the value of neutrophil to lymphocyte ratio in diagnosing coronary artery disease and its severity

摘要 目的研究中性粒细胞与淋巴细胞比值(NLR)在冠心病中的诊断价值。方法选取2017年5月-2019年5月在本院接受冠状动脉造影检查的患者(CAG)450例作为研究对象,根据CAG检查结果,分为冠心病组(CHD,245例)和非冠心病组(205例)两组。通过Gensini法对冠脉狭窄程度进行评估,分析NLR与冠心病狭窄程度相关性。另外通过绘制ROC曲线评估NLR预测冠心病诊断价值。结果两组在吸烟、高血压史差异具有统计学意义(P<0.05),年龄、性别、BMI、糖尿病、高血脂、TC、TG、HDL-C、LDL-C、apo A、apo B、Cr、UA、HCY、TBIL差异不具有统计学意义(P<0.05);NLR是冠心病独立危险因素[OR=5.545,95%CI(3.796-8.099),P<0.05]。Gensini评分与NLR水平呈正相关(r=0.376,P<0.01)。ROC曲线评估NLR=1.64,诊断冠心病敏感度=0.807,特异度=0.524,曲线下面积=0.756,P<0.05。结论NLR是冠心病一个独立预测因素,且于其狭窄程度呈相关性。NLR对诊断冠心病有一定的诊断价值。 Objective To study the diagnostic value of neutrophil to lymphocyte ratio(NLR)with in coronary artery disease.Methods 450 patients received coronary angiography(CAG)were randomly selected and245 cases were divided into coronary heart disease group(CHD)and 205 cases in non-coronary heart disease group according to the results of CAG.Gensini method was used to evaluate the degree of coronary stenosis,and the correlation between NLR and the degree of coronary stenosis was analyzed.In addition,the diagnostic value of NLR in predicting coronary heart disease was evaluated by drawing ROC curve.Results There were significant differences in smoking and hypertension history between the two groups(P<0.05).And there were no significant differences in Age,sex,BMI,diabetes,hyperlipidemia,TC,TG,HDL-C,LDL-C,apo A,apo B,Cr,UA,HCY and TBIL(P<0.05).NLR was an independent risk factor for coronary heart disease(OR=5.545,95%CI(3.796-8.099),P<0.05).Gensini score was positively correlated with NLR level(r=0.376,P<0.01).When NLR equal to 1.64,sensitivity was 0.807,specificity was 0.524.Area under curve=0.756,P<0.05.Conclusions NLR is an independent predictor of coronary heart disease,and is correlated with the degree of stenosis.NLR has certain diagnostic value in the diagnosis of coronary heart disease.
