摘要
目的探究品管圈活动对降低神经内科患者发生鼻胃管非计划拔管的效果。方法选取2018年9月至2019年6月在本院神经内科实施鼻胃管干预的260例患者,根据入院时间将患者分为两组,各130例。常规组患者行常规干预,研究组患者行品管圈活动干预,比较两组患者治疗期间非计划性拔管发生率及置管期间负性情绪。结果研究组非计划拔管率为6.15%,显著低于常规组21.54%,差异有统计学意义(P<0.05);研究组躯体化、强迫症状、人际关系、敏感、抑郁、敌对、恐怖、偏执、精神病性评分分别为(0.51±0.19)分、(0.02±0.01)分、(0.32±0.24)分、(0.65±0.29)分、(0.68±0.31)分、(0.57±0.35)分、(0.45±0.31)分、(0.17±0.01)分、(0.21±0.08)分,均低于常规组,差异均有统计学意义(均P<0.05)。结论对采用鼻胃管干预的神经内科患者实施品管圈活动干预,能够有效降低鼻胃管非计划拔管发生概率,同时有助于改善患者负性情绪,对促进患者康复,避免再次置管对患者造成的损伤具有重要意义。
Objective To investigate the effect of quality control circle on reducing the unplanned extubation of nasogastric tube in neurological patients.Methods A total of 260 patients who underwent nasogastric tube intervention in our department of neurology from September,2018 to June,2019 were enrolled.According to the admission time,the patients were divided into a routine group and a study group.The routine group was routinely intervened.The study group were intervened with quality control circle.The incidence of unplanned extubation and negative emotion during catheterization were compared between the two groups.Results The incidence of unplanned extubation was significantly lower in the study group than in the conventional group(6.15%vs.21.54%;P<0.05).The scores of somatization,compulsive symptoms,interpersonal relation,sensitivity,depression,hostility,horror,paranoid,and psychiatrics were(0.51±0.19),(0.02±0.01),(0.32±0.24),(0.65±0.29),(0.68±0.31),(0.57±0.35),(0.45±0.31),(0.17±0.01),and(0.21±0.08)in the study group,which were lower than those in the control group(all P<0.05).Conclusion The application of quality control circle in neurological patients with nasogastric tube can effectively reduce the incidence of unplanned extubation,and help improve their negative emotions and recovery and avoid re-intubation.
