胃癌患者术后生活体验质性研究的系统评价和Meta整合

Postoperative Life Experience of Patients with Gastric Cancer:a Systematic Review and Meta-Synthesis of Qualitative Studies

目的系统评价和整合胃癌患者术后的生活体验,为指导胃癌术后的康复护理提供依据。方法检索The Cochrane Library、Pubmed、EMbase、Web of Science、CINAHL、Elsevier SD、OVID、CMB、VIP、CNKI和万方数据库从建库至2018年12月的相关质性研究。采用“澳大利亚JBI循证卫生保健中心对质性研究的真实性评价”评价文献,采用汇集性整合方法整合结果。结果7项研究符合标准,整合出2大类别:1)饮食从快乐的生活享受变成被迫的营养治疗,但患者学会适应,更利于康复;2)患者经历了手术对生理、心理和社会的消极影响,渴求家庭社会支持,更加珍惜当下,含6个新类别,共29个结果。结论卫生保健人员和家庭应充分重视患者术后的饮食诉求和各种身心、家庭和社会不适反应及相应需求,并给予支持和关爱,提高患者应对术后生活的能力,以促进患者康复,延长患者寿命。

Objective To systematically review and integrate the postoperative life experience,and to provide a basis for guiding postoperative rehabilitation in patients with gastric cancer.Methods The Cochrane Library,Pubmed,EMbase,ISI Web of Science,CINAHL,Elsevier SD,OVID,CMB,CNKI,VIP and Wanfang Data were searched from inception to November 2018 to collect qualitative studies on postoperative life experience of patients with gastric cancer.The quality of included studies was evaluated according to JBI critical appraisal tool for qualitative studies in Australia.The results were integrated using convergent integration method.Results Totally 7 studies containing 29 complete findings were included.Then 6 new categories were generated and finally 2 synthesized results were obtained.Result l:Eating changed from enjoying a happy life to forced nutrition therapy,but patients learned to adapt and were more conducive to recovery.Result 2:Postoperative patients with gastric cancer had negative physical,psychological and social experiences,craved family and social support,and cherished the present more.Conclusion In order to promote recovery and prolong life in patients with gastric cancer,health care workers and families should pay full attention to patients’postoperative dietary appeal and various physical,mental,family and social discomforts and corresponding needs,provide support and care,and improve patients’ability to cope with postoperative life.
