摘要
目的观察慢性阻塞性肺疾病(COPD)患者应用百令胶囊治疗的效果,并探讨其对患者肺功能、T淋巴细胞亚群及免疫炎症因子的影响。方法选择上海市杨浦区中心医院2017年8月至2019年8月收治的COPD患者,按照随机数表法将患者分为观察组和对照组,每组39例。对照组患者给予常规的综合治疗,观察组患者在常规的综合治疗基础上给予百令胶囊治疗,连续治疗8周。治疗结束后,比较两组患者的治疗总有效率、肺功能、T淋巴细胞亚群(CD4^+、CD8^+、CD4^+/CD8^+、CD3^+)及白介素-6 (IL-6)、白介素-8 (IL-8)、转化生长因子β1(TGF-β1)、基质金属蛋白酶-9 (MMP-9)、组织基质金属蛋白酶抑制剂-1 (TIMP-1)等炎症因子水平。结果观察组患者的治疗总有效率为92.31%,明显高于对照组的71.80%,差异有统计学意义(P<0.05);治疗后,观察组和对照组患者的呼气峰值流速(PEF)[(59.13±6.11)%vs (55.78±5.61)%]、第1秒用力呼气容积(FEV1)[(1.81±0.45) L vs (1.59±0.40) L]、FEV1/FVC(用气肺活量)[(60.24±6.38)%vs (56.44±5.70)%]、CD4^+[(46.89±7.29) vs (37.01±6.01)]、CD4^+/CD8^+[(1.52±0.19) vs(1.02±0.14)]、CD3^+[(58.39±7.19) vs (43.79±7.39)]、TIMP-1[(154.48±9.32)μg/L vs (125.23±9.07)μg/L]等指标比较,观察组明显高于对照组,差异均有统计学意义(P<0.05);治疗后,观察组和对照组患者的CD8^+[(30.01±5.01) vs (36.01±5.31)]、TGF-β1[(115.35±15.63)μg/L vs (159.93±19.47)μg/L]、MMP-9 [(172.57±24.68)μg/L vs (203.53±28.36)μg/L]、IL-6 [(65.87±25.42) pg/L vs (96.13±40.42) pg/L]、IL-8 [(275.76±102.42) ng/L vs (330.96±93.03) ng/L]等指标比较,观察组明显低于对照组,差异均具有统计学意义(P<0.05)。结论 COPD患者应用百令胶囊可提高患者的治疗效果,改善肺功能及免疫功能,降低患者的炎性反应,促进康复。
Objective To observe the effect of Bailing Capsule on patients with chronic obstructive pulmonary disease(COPD), and to explore its effects on lung function, T lymphocyte subsets, and immune inflammatory factors.Methods COPD patients admitted to Shanghai Yangpu District Central Hospital from August 2017 to August 2019 were selected, and the patients were divided into observation group and control group according to random number table method, with 39 patients in each group. Patients in the control group were given conventional comprehensive treatment,and patients in the observation group were given Bailing capsules for 8 weeks. After treatment, the two groups were compared for total effective rate, lung function, T lymphocyte subsets(CD4^+, CD8^+, CD4^+/CD8^+, CD3^+), interleukin-6(IL-6), interleukin-8(IL-8), transforming growth factor β1(TGF-β1), matrix metalloproteinase-9(MMP-9), tissue matrix metalloproteinase inhibitor-1(TIMP-1), and other inflammatory factors. Results The total effective rate of treatment in the observation group was 92.31%, which was significantly higher than 71.80% in the control group(P<0.05).After treatment, peak expiratory flow rate(PEF), forced expiratory volume in the first second(FEV1), forced expiratory volume/expiratory vital capacity in the first second(FEV1/FVC), CD4^+, CD4^+/CD8^+, CD3^+, TIMP-1 in the observation group were significantly higher than those in the control group(P<0.05): PEF,(59.13±6.11)% vs(55.78±5.61)%;FEV1,(1.81±0.45) L vs(1.59±0.40) L;FEV1/FVC,(60.24±6.38)% vs(56.44±5.70)%;CD4^+,(46.89±7.29) vs(37.01±6.01);CD4^+/CD8^+,(1.52±0.19) vs(1.02±0.14);CD3^+,(58.39±7.19) vs(43.79±7.39);TIMP-1,(154.48±9.32) μg/L vs(125.23±9.07) μg/L. After treatment, CD8^+, TGF-β1, MMP-9, IL-6, IL-8 in the observation group were significantly lower than those in the control group(P<0.05): CD8^+,(30.01 ± 5.01) vs(36.01 ± 5.31);TGF-β1,(115.35 ± 15.63) μ g/L vs(159.93 ±19.47) μg/L;MMP-9,(172.57±24.68) μg/L vs(203.53±28.36) μg/L;IL-6,(65.87±25.42) pg/L vs(96.13±40.42) pg/L;IL-8,(275.76 ± 102.42) ng/L vs(330.96 ± 93.03) ng/L. Conclusion Application of Bailing capsule in patients with COPD can improve the treatment effect of patients, improve lung function and immune function, reduce patients’ inflammatory response and promote rehabilitation.
