关节镜下微创踝关节融合术治疗足踝创伤性关节炎的疗效分析

Therapeutic Effect of Arthroscopic Minimally Invasive Ankle Arthrodesis in the Treatment of Traumatic Arthritis of Athlete's Foot

目的探讨关节镜下微创踝关节融合术治疗足踝创伤性关节炎的疗效。方法方便选择2015年1—12月在该院住院治疗的足踝创伤性关节炎患者120例,按照随机数表法分为两组,各60例。实验组采用关节镜下微创踝关节融合术治疗,对照组采用常规开放式踝关节融合术治疗。观察两组患者治疗前、后6个月的踝关节Kofoed评分比较其术后6个月及1年的骨性融合率及不良反应发生情况。结果两组患者治疗后踝关节Kofoed评分(78.51±6.57,67.30±5.34)分均高于治疗前(18.32±2.33,18.44±2.32)分,且观察组(78.51±6.57)分高于对照组(67.30±5.34)分,差异有统计学意义(t1=46.240,P=0.000;t2=34.853,P=0.000;t3=0.298,P=0766;t4=7.285,P=0.000)。对照组患者术后6个月及1年的骨性融合率低于观察组(25.00%vs58.33%,71.66%vs93.33%),差异有统计学意义(χ^2=12.377,P=0.000;χ^2=8.312,P=0.004)。结论关节镜下踝关节融合术能够显著改善创伤性踝关节炎患者治疗效果,值得临床推广应用。

Objective To evaluate the efficacy of arthroscopic minimally invasive ankle arthrodesis in the treatment of traumatic arthritis of the foot.Methods A total of 120 patients with traumatic arthritis of the foot and ankle treated in the hospital from January-December 2018 were convenient enrolled.According to the random number table method,60 patients were divided into two groups.The experimental group was treated with arthroscopic minimally invasive ankle arthrodesis,and the control group was treated with conventional open ankle arthrodesis.The Kofoed scores of the ankle joints before and after treatment were compared between the two groups.The bone fusion rate and adverse reactions occurred 6 months and 1 year after operation.Results The Kofoed scores of the ankle joints after treatment(78.51±6.57,67.30±5.34)were higher than those before treatment(18.32±2.33,18.44±2.32)points,and the observation group(78.51±6.57)points was higher than the control group(67.30±5.34),the difference was statistically significant(t1=46.240,P=0.000;t2=34.853,P=0.000;t3=0.298,P=0766;t4=7.285,P=0.000).The rate of bone fusion in the control group at 6 months and 1 year was lower than that in the observation group(25.00%vs 58.33%;71.66%vs 93.33%),the difference was statistically significant(χ^2=12.377,P=0.000;χ^2=8.312,P=0.004).Conclusion Arthroscopic ankle arthrodesis can significantly improve the therapeutic effect of patients with traumatic ankle arthritis,which is worthy of clinical application.
