伴IgA沉积的抗肾小球基底膜病1例报告并文献复习

A Case Report of Anti-glomerular Basement Membrane Disease with IgA Deposition and Literature Review

目的探讨合并IgA沉积的抗肾小球基底膜病临床与病理特点。方法将该院收治的伴IgA沉积的抗肾小球基底膜病病例1例,结合文献复习近3年来报道的伴IgA沉积的抗肾小球基底膜病病例10例,与文献中经典抗肾小球基底膜病病例53例进行对照。结果伴IgA沉积的抗肾小球基底膜病在临床、病理表现存在一定差异,但治疗与转归与经典抗肾小球基底膜病无明显差异。结论合并IgA沉积的抗GBM病可能与经典抗GBM病发病机制存在差异。及时的肾活检及检测血清抗GBM抗体,以减少误漏诊,早期诊断,早期治疗,改善预后。

Objective To investigate the clinical and pathological features of anti-glomerular basement membrane disease with IgA deposition.Methods One case of anti-glomerular basement membrane disease with IgA deposition in the hospital was combined with literature review of 10 cases of anti-glomerular basement membrane disease with IgA deposition reported in the past 3 years.53 cases of glomerular basement membrane disease were compared.Results There was a difference in clinical and pathological manifestations of anti-glomerular basement membrane disease with IgA deposition,but there was no significant difference between treatment and outcome and classical anti-glomerular basement membrane disease.Conclusion The anti-GBM disease combined with IgA deposition may be different from the pathogenesis of classical anti-GBM disease.Timely renal biopsy and detection of serum anti-GBM antibodies can reduce false missed diagnosis,early diagnosis,early treatment can help to improve prognosis.
