成人OSAHS治疗中扁桃体切除术联合悬吊缝合术的应用效果探讨

Discussion on the Effect of Tonsillectomy Combined with Suspension Suture in the Treatment of Adult OSAHS

目的探讨成人阻塞性睡眠呼吸暂停低通气综合征(OSAHS)治疗中扁桃体切除术联合悬吊缝合术的应用近期疗效。方法方便选择该院2016年1月-2018年1月期间收治的104例OSAHS(均为Friedman I期)患者作为研究对象,按照患者治疗所采取手术方法的不同分为研究组和对照组,其中研究组患者均采用扁桃体切除术联合悬吊缝合术治疗,对照组则采用悬雍垂腭咽成形术治疗,统计两组患者手术及并发症情况,并比较AHI、LSaO2、ESS等指标变化。结果对照组并发症发生率为7.69%,研究组并发症发生率为1.92%,对照组患者并发症发生率略高于研究组,但差异无统计学意义(P>0.05);研究组患者治疗前平均AHI为(43.5±7.4)次/h,治疗后平均AHI为(12.6±5.7)次/h,组内治疗前后数据比较差异有统计学意义(t=23.855,P=0.000);对照组患者治疗前平均AHI为(43.3±7.6)次/h,治疗后平均AHI为(12.9±6.1)次/h,组内治疗前后数据比较差异有统计学意义(t=22.495,P=0.000),但两组间治疗前及治疗后数据比较,差异无统计学意义(P>0.05)。结论成人OSAHS治疗中应用扁桃体切除术联合悬吊缝合术治疗具有操作简单、创伤小、安全性高、治疗效果满意等特点,有助于改善患者睡眠及生活质量,值得推广。

Objective To explore the short-term efficacy of adult obstructive sleep apnea hypopnea syndrome(OSAHS) in the treatment of middle tonsillectomy combined with suspension suture. Methods From January 2016 to January 2018 treated 104 cases of OSAHS(both for Friedman stage I) patients as the research object, according to the patients by surgical method convenient divided into research group and the control group, the group patients adopt tonsil resection for the treatment of combined suspension suture, control group adopts the uvula palate pharyngoplasty treatment, statistics and complications of two groups of patients with surgery, and compare the AHI, LSaO2, ESS and other indicators. Results The complication rate was 7.69% in the control group and 1.92% in the study group. The complication rate in the control group was slightly higher than that in the study group, but the difference was not statistically significant(P>0.05). The mean AHI was(43.5±7.4) times/h, and the mean AHI after treatment was(12.6±5.7) times/h. There was significant difference in the data before and after treatment(t=23.855, P=0.000). The mean AHI before treatment was(43.3±7.6) times/h, and the mean AHI after treatment was(12.9±6.1) times/h. Conclusion Tonsillectomy combined with suspension suture in adult OSAHS treatment is characterized by simple operation, small trauma.It helps to improve patients’ sleep and quality of life, and is worth promoting.
