崔文成治疗小儿变应性鼻炎肺虚食滞胃热证经验

Cui Wen-cheng′s Experience of Treating Allergic Rhinitis in Children with Syndrome of Lung Deficiency and Food Retention and Stomach Heat

崔文成教授提出小儿变应性鼻炎多为肺虚食滞胃热证。小儿变应性鼻炎肺虚食滞胃热证的内因:一是内有肺虚特禀体质,二是鼻有湿邪伏遏;外因为"非时之感",六淫、异气、情志、饮食均为非时之感。崔教授认为,本病病位在鼻,病变脏腑在肺,涉及肝、脾与胃肠,病性为本虚标实,肺气虚为本,饮食聚为标。其基本病机为内有肺虚特禀体质,外有非时之感,鼻有湿邪伏遏,三者相合,闭据鼻窍则发病。崔教授常用《济生方》苍耳子散、《医宗金鉴》平胃散和《丹溪心法》保和丸加减治疗,并注重小儿的预防调护,将"军事化治理"的理念告知家长,使之运用于小儿的日常调理,双管齐下,收效显著。"军事化治理"有五个要点:①起居有常有定时;②饮食有节有规律;③适度锻炼;④调畅情志;⑤全家总动员。

Professor Cui Wen-cheng has pointed out that children′s allergic rhinitis mainly has the syndrome of lung deficiency and food retention and stomach heat.The internal causes of it have two:One has special constitutional lung deficiency;the other one is that there is latent dampness pathogen in the nose.The external cause of it is due to"unseasonal pathogens",such as six pathogenic factors,abnormal qi,emotions and diets.He thinks that the location of disease is in the nose;the diseased organ is in the lung,involving the liver,spleen,stomach and intestines;the nature of disease is deficient root and excessive branch with lung-qi deficiency as the root and the food retention as the branch.So the basic disease mechanism is combination of internal special constitutional lung deficiency,external unseasonal pathogens,latent dampness in the nose,which blocks the nasal orifice and leads to allergic rhinitis.Professor Cui often chooses Fructus Xanthii Powder in Prescriptions to Aid the Living,Stomach-Calming Powder in Golden Mirror of the Medical Ancestors,and Harmony-Preserving Pill in Dan-xi′s Experiential Therapy for modification to treat allergic rhinitis in children.He also pays attention to prevention and adjustment,and informs the parents of the concept of"military governance",which should be applied to children′s daily conditioning,and has achieved remarkable results.There are five key points of"military management":①regular living;②regular diet;③moderate exercise;④smooth emotion;⑤general mobilization of the whole family.
