中药对子宫内膜异位症合并妊娠患者中医体质和焦虑抑郁状态的影响

Study on the TCM Constitution and Anxiety and Depression of Endometriosis with Pregnancy Treated with Chinese Medicinals

目的:观察中药对子宫内膜异位症合并妊娠患者中医体质和焦虑抑郁状态的影响。方法:选择84例子宫内膜异位症合并妊娠,服用中药治疗并有活产结局的患者为中药组,同时选择正常妊娠(非子宫内膜异位症),并有活产结局的90例产妇为对照组。分析两组患者的体质类型,比较两组患者孕前、孕期、产后中医体质变化特点、焦虑自评量表(self-rating anxiety scale,SAS)评分和抑郁自评量表(self-rating depression scale,SDS)评分。结果:中药组阳虚质39例、气虚质25例、阴虚质5例、痰湿质3例、湿热质2例、血瘀质4例、特禀质1例、气郁质3例、平和质2例。对照组阳虚质11例、阴虚质5例、气虚质10例、痰湿质4例、湿热质5例、血瘀质7例、特禀质3例、气郁质16例、平和质29例。两组患者孕期及产后阳虚质、气虚质转化分比较,差异具有统计学意义(P<0.05)。两组患者阳虚质、气虚质、气郁质、血瘀质及平和质的体质转化总分比较,差异具有统计学意义(P<0.05)。两组患者孕前、孕期阳虚质、气虚质、气郁质、平和质比较,差异有统计学意义(P<0.05);产后气郁质、平和质比较,差异有统计学意义(P<0.05)。两组患者孕期及产后SDS评分与本组孕前比较,差异具有统计学意义(P<0.05)。中药组焦虑状态及抑郁状态发生率高于对照组,差异有统计学意义(P<0.05)。结论:中药可改变子宫内膜异位症合并妊娠患者的中医体质及焦虑状态、抑郁状态。

Objective:To observe the influence of Chinese medicinals on the TCM constitution and anxiety and depression of endometriosis with pregnancy.Methods:A total of 84 patients of endometriosis with pregnancy treated with Chinese medicinals and having live birth outcome were selected as the Chinese medicinals group and 90 cases of normal pregnancy(non endometriosis)with live birth outcome as the control group.The constitutional types of the two groups were analyzed.The characteristics of the physical changes of TCM before,during and after pregnancy,the scores of Self-rating Anxiety Scale(SAS)and Self-rating Depression Scale(SDS)were compared.Results:In the Chinese medicinals group,there were 39 cases with yang deficiency constitution,25 with qi deficiency constitution,5 with yin deficiency constitution,3 with phlegm-dampness constitution,2 with damp-heat constitution,4 with blood stasis constitution,1 with special constitution,3 with qi depression constitution,and 2 with mild constitution.In the control group,there were 11 cases with yang deficiency constitution,10 with qi deficiency constitution,5 with yin deficiency constitution,4 with phlegm-dampness constitution,5 with damp-heat constitution,7 with blood stasis constitution,3 with special constitution,16 with qi depression constitution,and 29 with mild constitution.In terms of the transformation score,the differences in pregnancy and postpartum in yang deficiency constitution;qi deficiency constitution were statistically significant(P<0.05).In terms of the total transformation score,the differences in yang deficiency constitution,qi deficiency constitution,qi depression constitution,blood stasis constitution,and mild constitution were statistically significant(P<0.05).There were significant differences between the two groups before and during pregnancy in yang deficiency constitution,qi deficiency constitution,qi depression constitution,and mild constitution(P<0.05).There was significant difference in postpartum qi stagnation constitution and mild constitution(P<0.05).The difference of SDS scores in each groups before and after pregnancy was statistically significant(P<0.05).The incidence of anxiety and depression in the Chinese medicinals group was higher than that in the control group,and the difference was statistically significant(P<0.05).Conclusion:Chinese medicinals can improve the TCM constitution and anxiety and depression of patients with endometriosis with pregnancy.
