针刺结合督灸治疗中风后偏瘫临床研究

Clinical Study on Acupuncture Combined with Moxibustion at GV in the Treatment of Hemiplegia After Stroke

目的:观察针刺结合督灸治疗中风后偏瘫的临床效果。方法:选取2018年1月至2019年6月本院治疗的中风后偏瘫患者84例,随机分为观察组和对照组,每组42例。对照组给予降压、调脂、降糖、营养支持等对症治疗,观察组在对照组治疗的基础上给予针刺结合督灸治疗。结果:观察组有效率95.24%,对照组有效率78.57%,观察组高于对照组,差异有统计学意义(P<0.05);观察组治疗后各项中医证候积分均低于对照组,差异有统计学意义(P<0.05);观察组治疗后美国国立卫生研究院卒中量表评分、综合痉挛量表评分低于对照组,Fugl-Meyer运动功能评定量表评分、Berg平衡量表评分、日常生活能力评分高于对照组,差异有统计学意义(P<0.05);观察组治疗后髋关节最大屈曲角(maximum buckling angle,MBA)、髋关节最大伸展角(maximum extension angle,MEA)、髋关节屈伸角速度(angular velocity of buckling and extension,AV)、膝关节MBA、膝关节AV高于对照组,膝关节MEA、足偏角低于对照组,差异有统计学意义(P<0.05)。结论:针刺结合督灸治疗中风后偏瘫,能够有效改善患者神经功能与运动功能,促进患者康复。

Objective:To observe the clinical curative effect of acupuncture combined with moxibustion at GV on hemiplegia after stroke.Methods:A total of 84 patients with hemiplegia after stroke treated in our hospital from January 2018 to June 2019 were randomly divided into the observation group and the control group,with 42 in each group.The control group was given antihypertensive,lipid-lowering,hypoglycemic,nutritional support and other symptomatic treatment.The observation group was given acupuncture combined with moxibustion at GV on the basis of the treatment in the control group.Results:The effective rate was 95.24%in the observation group and 78.57%in the control group.The difference was statistically significant(P<0.05).After the treatment,the scores of TCM syndromes in the observation group were lower than those in the control group,and the difference was statistically significant(P<0.05).After the treatment,the scores of stroke scale and comprehensive spasm scale of NIH in the observation group were lower than those in the control group,and the scores of Fugl Meyer motor function rating scale,Berg Balance Scale and daily living ability were higher than those in the control group,and all the differences were statistically significant(P<0.05).After the treatment,the maximum flexion angle(MBA),the maximum extension angle(MEA),the angular velocity of flexion and extension(AV),the MBA and AV of the knee in the observation group were higher than those in the control group,and the MEA and foot deflection angle of the knee were lower than those in the control group,and all the differences were statistically significant(P<0.05).Conclusion:In terms of hemiplegia after stroke,acupuncture combined with moxibustion at GV can effectively improve the neurological and motor functions of patients,and promote their rehabilitation.
