针刀浅筋膜松解术联合神经根阻滞治疗腰椎间盘突出症的临床研究

Clinical study on the treatment of lumbar disc herniation by acupotomy and nerve root block

目的:观察针刀浅筋膜松解术联合神经根阻滞治疗腰椎间盘突出症(lumbar disc herniation,LDH)的临床疗效。方法:选取望京医院门诊LDH患者60例按随机数字表法分为治疗组(针刀浅筋膜松解术+神经根阻滞术)和对照组各30例(神经根阻滞术)。于术前术后3 d、术后2周记录患者VAS评分以及Oswestry功能障碍指数(ODI)。结果:2组的VAS评分以及治疗组ODI指数在上述3个时间点均逐渐减低,组内比较差异有统计学意义;治疗组术后2周VAS评分和ODI指数均低于对照组,差异有统计学意义;治疗组与对照组ODI指数的有效率分别为86.67%和63.33%。结论:在改善疼痛程度及腰椎活动功能方面,针刀浅筋膜松解+神经根阻滞术治疗LDH比单用神经根阻滞术的疗效优势明显。

Objective:To observe the clinical effect of superficial fascial release with needle knife combined with nerve root block in the treatment of Lumbar Disc Herniation(LDH).Methods:60 patients with LDH who came to Wangjing Hospital of Chinese Academy of Chinese Medical Sciences were randomly divided into treatment group(superficial fascial release with needle knife combined with nerve root block)and control group(single nerve root block),30 cases in each group.The VAS score and Oswestry Dysfunction Index(ODI)were recorded before operation,3 days after operation,and 2 weeks after operation.The effective rate of ODI was evaluated.Results:The VAS score and ODI of the two groups decreased gradually at the above three time points,and there was significant statistical difference in both groups.The VAS score and ODI in 2 weeks after operation were significantly different between the two groups.The effective rate of ODI in the treatment group was 86.67%,compared with 63.33%in the control group.Conclusion:The superficial fascial release with needle knife combined with nerve root block is more effective than single nerve root block on improving pain and lumbar function for LDH.
