胃癌术后患者早期肠内营养支持的临床应用

Clinical application of early enteral nutrition support in patients undergoing surgery for gastric carcinoma

目的观察胃癌术后患者早期肠内营养支持的临床应用效果。方法选取2016年9月~2018年9月我院胃癌术后患者120例,随机分为早期肠外营养支持的对照组60例与早期肠内营养支持的观察组60例,观察两组患者的恢复情况、营养指标及并发症发生率。结果观察组排气时间(3.51±1.16)d,排便时间(4.28±0.42)d,住院时间(11.36±2.16)d,分别少于对照组,P<0.05;术前,两组营养指标对比,P>0.05,术后7 d,两组营养指标均较术前有了较大改善,P<0.05,且观察组白蛋白(35.72±1.59)g/L,前白蛋白(256.37±1.35)mg/L,血红蛋白(90.73±9.74)g/L,分别显著高于对照组,P<0.05;观察组(5.00%)并发症发生率显著低于对照组(16.67%),P<0.05。结论给予胃癌术后患者早期肠内营养支持,能促进患者身体恢复,有效改善其营养情况,降低并发症发生率,效果较好,值得借鉴。

Objective To investigate the clinical efficacy of early enteral nutrition(EN)support in patients undergoing surgery for gastric carcinoma.Methods A total of 120 patients experiencing surgery for gastric carcinoma at our hospital from September 2016 to September 2018 were selected.They were randomly divided into the control group(n=60)and the observation group(n=60).Patients in the control group received early parenteral nutrition(PN)support,and those in the observation group accepted early EN support.Post-operative recovery,nutrition indices and incidence rates of complications in all patients were observed.Results Time to the first flatus(3.51±1.16)days and defecation(4.28±0.42)days and length of hospital stay(11.36±2.16)days in the observation group were shorter than those in the control group(P<0.05).There were no significant differences in the baseline nutrition indices between the two groups(P>0.05).However,nutrition indices were improved in the two groups at 7 days after surgery(P<0.05),and levels of albumin(35.72±1.59)g/L,prealbumin(256.37±1.35)mg/L and hemoglobin(90.73±9.74)g/L in the observation group were higher than those in the control group(P<0.05).The incidence rate of complications in the observation group was 5.00%,lower than that of 16.67%in the control group(P<0.05).Conclusion Early EN support facilitated post-operative recovery in patients undergoing surgery for gastric carcinoma,with better nutrition states and a lower incidence rate of complications.As early EN support has achieved favorable clinical efficacy,it is worth learning from in clinical practices.
