骨质疏松性椎体骨折不同时期MRI表现腰背痛严重程度的相关性分析

Correlation analysis of the severity of low back pain and MRI images at different stages of osteoporotic vertebral fracture

目的通过MRI评价骨质疏松性椎体骨折(osteoporotic vertebral fractures,OVF)表现和腰背痛严重程度之间的相关性。方法本研究纳入2014年5月-2018年10月年我院骨科就诊的20例骨质疏松椎体骨折患者,分别在入组时、1个月后、3个月后进行MRI检查和背痛VAS评分。在每个时间点获得T1和T2加权图像(T1WI和T2WI),并分析骨折图像与不同时间点背痛视觉模拟评分(visual analogy score,VAS)的相关性。结果 20例中,2例失访,共18例患者完成了为期3个月的随访。入组时,背痛的平均VAS评分为(75.2±11.3)分,在3个月后疼痛明显改善,平均VAS评分为(22.5±7.1)分。在入组及1个月随访时,不同T1WI及T2WI信号患者的VAS评分差异均无统计学意义(P>0.05);3个月随访时,不同T1WI及T2WI信号患者的VAS评分差异均有显著统计学意义(P<0.05)。结论 OVF患者在不同时期的MRI影像学改变与腰背痛的严重程度之间存在显著相关性。

Objective To evaluate the correlation between MRI images of osteoporotic vertebral fractures (OVF) and the severity of low back pain.Methods In this study,20 patients with OVF admitted to the department of orthopedics in our hospital from 2014-October 2018 were included.MRI examination and VAS score for back pain were performed at the time of enrollment,1 month later and 3 months later.T1 and T2-weighted images (T1WI and T2WI) were obtained at each time point,and the correlation between fracture images and visual analogue back pain scores (VAS) at different time points was analyzed.Results Of the 20 patients,2 cases were lost to follow-up,and a total of 18 patients completed a 3-month follow-up.The average VAS score of back pain was (75.2±11.3) at the time of admission.After 3 months,the pain was significantly improved,the average VAS score was (22.5±7.1).There was no significant difference in VAS score between patients with different T1WI and T2WI signals at the time of admission and 1 month follow-up (P>0.05).There was significant difference in VAS score between patients with different T1WI and T2WI signals at the time of 3 months follow-up (P<0.05).Conclusion There is a significant correlation between the changes of MRI images and the severity of low back pain in OVF patients at different period.
