IL-1、IL-6在椎间盘源性腰痛患者椎间盘退变中的作用研究

Role of IL-1 and IL-6 in water loss and degeneration of intervertebral disc nucleus pulposus in patients with intervertebral discogenic lumbago

目的探讨IL-1、IL-6在椎间盘源性腰痛患者椎间盘退变中的作用。方法选择本院2016年3月-2017年12月收治的30例椎间盘源性腰腿痛患者,行常规MRI检查,测定T2相对值(T2ratio)。按照JOA评分标准对患者的主观症状、体征、日常生活动作、膀胱功能进行评定。所有患者进行腰椎后路椎间盘髓核切除术,术中取出穿刺所得椎间盘,测定椎间盘组织中IL-1、IL-6浓度。分析T2ratio与IL-1、IL-6、JOA评分的相关性。结果 IL-1和IL-6水平与T2ratio呈显著负相关(r=-0.781和-0.852,P=0.000);T2ratio与JOA评分呈显著正相关(r=0.764,P=0.000)。结论 IL-1、IL-6在椎间盘源性腰痛患者椎间盘退变的过程中发挥重要作用,与疼痛症状的发生有明显相关性。

Objective To explore the role of IL-1 and IL-6 in water loss and degeneration of intervertebral disc nucleus pulposus in patients with intervertebral discogenic lumbago.Methods From March 2016 to December 2017,30 patients with intervertebral discogenic lumbago admitted to our hospital were selected in this study.Conventional lumbar MRI was performed to determine the relative value of T2 (T2ratio).Subjective symptoms,physical signs,daily life movements and bladder function were assessed according to Japanese Orthopaedic Association (JOA) evaluation criteria.All patients underwent posterior lumbar intervertebral disc nucleus pulpotomy,and the intervertebral disc obtained by puncture was removed.The concentrations of IL-1 and IL-6 in intervertebral disc tissues were determined.The correlation between T2ratio and IL-1,IL-6,JOA scores was analyzed.Results IL-1 and IL-6 levels were negatively correlated with T2ratio (r =-0.781 and-0.852,P = 0.000).The T2ratio was positively correlated with JOA score (r = 0.764,P = 0.000).Conclusion IL-1 and IL-6 play important roles in the process of intervertebral disc degeneration in patients with discogenic lumbago,which are significantly correlated with the occurrence of pain symptoms.
