缝线吻合技术在鼻内窥镜下泪囊鼻腔吻合术的临床应用

Clinical application of suture anastomosis technique in nasal endoscopic dacryocystorhinostomy

目的探究缝线吻合技术在鼻内窥镜下泪囊鼻腔吻合术的临床应用。方法回顾性分析2018年1月至2019年4月在本院行鼻内窥镜下泪囊鼻腔吻合术治疗的100例鼻泪管阻塞性疾病患者的临床资料,根据吻合方法不同分为对照组(n=50)和观察组(n=50)。对照组采用不缝线吻合技术(明胶海绵填塞),观察组采用缝线吻合技术。观察两组术后疗效、吻合口黏膜上皮愈合时间、术后吻合口区瘢痕增生及并发症发生情况。结果观察组总有效率高于对照组,差异有统计学意义(P<0.05);观察组吻合口黏膜上皮愈合时间低于对照组,差异有统计学意义(P<0.05);观察组并发症发生率及吻合口区瘢痕增生发生率均低于对照组,差异有统计学意义(P<0.05)。结论缝线吻合技术应用于鼻内窥镜下泪囊鼻腔吻合术中,可有效降低泪道阻塞性疾病患者术后并发症及瘢痕增生发生率,缩短术后吻合口愈合时间,提高手术成功率。

Objective To investigate clinical application of suture anastomosis technique in nasal endoscopic dacryocystorhinostomy.Methods The clinical data of 100 naso-lacrimal duct obstruction patients with nasal endoscopic dacryocystorhinostomy in the hospital from January 2018 to April 2019 were retrospectively analyzed and they were divided into control group(n=50)and observation group(n=50)according to different anastomosis methods.Control group was given non-stitch anastomosis technique(gelatin sponge packing),and observation group was given suture anastomosis technique.The postoperative efficacy,healing time of anastomotic mucosal epithelium,postoperative scar proliferation in anastomosis area,and complications in two groups were observed.Results The total effective rate of observation group was higher than that of control group(P<0.05).The healing time of anastomotic mucosal epithelium in observation group was lower than that in control group(P<0.05).The incidence of complications and the scar proliferation in anastomosis area in observation group was lower than that in control group(P<0.05).Conclusion Suture anastomosis technique in nasal endoscopic dacryocystorhinostomy can decrease the incidence of postoperative complications and scar proliferation in patients with lacrimal duct obstruction diseases,shorten postoperative anastomosis healing time,and increase the success rate of surgery.
