子宫肌瘤剔除术后残留和复发的临床危险因素分析

Analysis of clinical risk factors for residual and recurrence of uterine leiomyoma after myomectomy

目的探究子宫肌瘤剔除术后残留和复发的临床危险因素。方法选取2016年1月至2018年12月在本院接受子宫肌瘤剔除术治疗的子宫肌瘤患者110例,观察术后的残留和复发情况,并对相关危险因素进行分析。结果110例患者子宫肌瘤剔除术后残留复发率为27.27%,其中残留率为20.00%,复发率为7.27%;不同年龄、肌瘤数目、肌瘤位置、肌瘤直径、肌瘤类型的患者术后残留复发率对比差异具有统计学意义(P<0.05);多因素Logistic回归分析表明,子宫肌瘤剔除术后残留复发的相关危险因素主要为发病年龄、肌瘤位置、肌瘤数目、肌瘤直径以及肌瘤类型(P<0.05)。结论发病年龄、肌瘤位置、肌瘤数目、肌瘤直径以及肌瘤类型是子宫肌瘤剔除术后残留和复发的主要临床危险因素,临床医师应根据上述危险因素加强术前检查和评估,并实施相应的防控措施,以降低术后残留和复发率。

Objective To study the clinical risk factors of residual and recurrence of uterine leiomyoma after myomectomy.Methods 110 patients with uterine leiomyoma who underwent hysteromyomectomy in our hospital from January 2016 to December 2018 were included in this study.The residual and recurrence of uterine leiomyoma after operation were counted,and the related risk factors were analyzed.Results The residual recurrence rate was 27.27% in 110 patients after myomectomy,in which the residual rate was 20.00% and the recurrence rate was 7.27%.There was significant difference in the residual and recurrence rates among patients of different ages,number of myomas,location of myomas,diameter of myomas and types of myomas(P<0.05).Multivariate Logistic regression analysis showed that the main risk factors for residual recurrence after myomectomy were age,location of myoma,number,diameter and type of myoma(P<0.05).Conclusion Age,location of myoma,number of myoma,diameter of myoma and type of myoma are the main clinical risk factors for residual and recurrence of uterine leiomyoma after myomectomy.Clinicians should strengthen preoperative examination and evaluation according to the above risk factors,and implement corresponding prevention and control measures to reduce the rate of residual and recurrence after myomectomy.
