不同禁饮时间对胃食管反流合并症手术患者围手术期的影响

Effect of different duration of drink abstinence on perioperative period of patients with gastroesophageal reflux

目的探讨不同禁饮时间对胃食管反流合并症手术患者的影响。方法选择胆囊结石合并胃食管反流需行腹腔镜胆囊切除术的168例患者,按照术前不同的禁饮时间将所有患者用随机数字表随机分为A(3 h)、B(4 h)、C(5 h)、D(6 h)四个组,比较各组手术前后口渴、饥饿、焦虑、疼痛得分和术后下床时间、排气排便时间、恶心、呕吐等情况。结果手术前后A组(3 h)口渴、饥饿和术后疼痛得分都少于D组(6 h),差异具有统计学意义(P<0.05)。术后其他指标对比发现A组(3 h)与D组(6 h)在下床时间、排气排便、进食时间、停止输液时间和恶心的发生方面差异均具有统计学意义(P<0.05),但是各组之间焦虑差异无统计学意义(P>0.05)。结论缩短术前禁饮时间,可以减少患者手术前后口渴、饥饿感,增加患者术后恢复,减轻患者手术应激反应。

Objective To investigate the effect of different duration of drink abstinence on patients with gastroesophageal reflux.MethodsA total of 168 cases of cholecystolithiasis with gastroesophageal reflux requiring laparoscopic cholecystectomy were selected,according to the different forbidden drink time all of the patients with preoperative random number table were randomly divided into A(3 h),B(4 h),C(5 h)and D(6 h)groups,with comparison before and after surgery thirst,hunger,anxiety,pain scores and bed time,postoperative exhaust time defecate,nausea,vomiting,and so on between the groups.ResultsThe scores of thirst,hunger and postoperative pain in the group A were lower than those in the group D(P<0.05).Postoperative comparison of other indicators showed that there were statistically significant differences between the group A and group D in bed time,exhaust and defecation time,feeding time,infusion stop time and incidence of nausea(P<0.05),but there was no statistically significant difference in anxiety between each group(P>0.05).ConclusionShortening the duration of drinking prohibition before and after surgery can reduce patients′thirst and hunger,increase patients′postoperative recovery,and reduce patients′surgical stress response.
