抗苗勒管激素和窦卵泡计数对卵巢功能正常患者卵巢反应性的预测价值 被引量:14

The value of serum anti-Mullerian hormone(AMH)and antral follicle count(AFC)for ovarian response in patients with normal ovarian function

摘要 目的探讨对于卵巢功能正常患者,血清抗苗勒管激素(anti-Mullerian hormone,AMH)和窦卵泡计数(antral follicle count,AFC)预测卵巢反应性的价值。方法回顾性分析2016年1月至2018年11月在我院首次接受辅助生殖助孕治疗的患者567例,根据卵巢反应性分为低反应组(n=35)、正常反应组(n=421)和高反应组(n=111),采用受试者工作特征曲线(ROC)分析血清AMH和AFC与卵巢反应性的关系,并判断其预测卵巢反应性的截点值。结果血清AMH和AFC可以作为预测卵巢反应性的独立指标,AMH和AFC预测卵巢低反应的曲线下面积分别为0.805、0.796,截点值分别为2.51 ng/mL、11个。AMH和AFC预测高反应的曲线下面积分别为0.706、0.740,截点值分别为4.755 ng/mL、15个。结论对于卵巢功能正常的患者,血清AMH和AFC是卵巢反应性的有效独立预测因子,应根据本中心数据得出其预测卵巢反应性的截点值。 Objective To investigate the value of serum anti-Mullerian hormone(AMH)and antral follicle count(AFC)for ovarian response in patients with normal ovarian function.Methods 567 patients were retrospective analyzed in our center from January 2016 to November 2018.These patients were divided into low(n=35),normal(n=421)and high(n=111)response groups according to the ovarian response.The cut-off points of AMH and AFC for predicting ovarian response were evaluated with the receiver operating characteristic curve.Results Serum AMH and AFC could be used as independent predictor of ovarian response.The area under the receiver operating characteristic curve(AUC)analysis of AMH and AFC for low ovarian response prediction was 0.805 and 0.796,respectively,and the cut-off point of AMH and AFC was 2.51 ng/mL and 11,respectively.The AUC analysis of AMH and AFC for high ovarian response prediction was 0.706 and 0.740,respectively,and the cut-off point of AMH and AFC was 4.755 ng/mL and 15,respectively.Conclusion Serum AMH and AFC are effective and independent predictors of ovarian response in patients with normal ovarian function.The cut-off points of predicting ovarian response should be based on our data.
