外固定支架治疗开放性SchatzkerⅤ、Ⅵ型 胫骨平台骨折的疗效

Effect of External Fixation on Open SchatzkerⅤandⅥType Tibial Plateau Fractures

目的分析外固定支架治疗开放性SchatzkerⅤ、Ⅵ型胫骨平台骨折的疗效。方法回顾性分析2015年1月~2018年6月我院收治的开放性schatzkerⅤ、Ⅵ型胫骨平台骨折患者28例,均行外固定支架治疗。根据Rasmussen影像学评分对骨折复位进行评估,采用末次随访时HSS评分对膝关节功能进行评估。结果术后首次平片的Rasmussen影像学评分:优7例,良19例,可2例,评分9~18分,平均(15.32±2.08)分。24例6月内达到骨愈合,4例延迟愈合,取髂骨植骨术后愈合,愈合率100.00%。末次随访时膝关节HSS评分:优7例,良15例,可4例,差2例,评分47~93分,平均(78.81±9.41)分。结论在开放性SchatzkerⅤ、Ⅵ型胫骨平台骨折中应用外固定架治疗复位质量及愈合良好,可有效改善膝关节功能,疗效满意。

Objective To analyze the effect of external fixation on open SchatzkerⅤandⅥtype tibial plateau fractures.Methods A retrospective analysis of 28 patients with open schatzkerⅤandⅥtibial plateau fractures admitted to our hospital from January 2015 to June 2018 were all treated with external fixation.The fracture reduction was evaluated based on the Rasmussen imaging score,and the HSS score at the last follow-up was used to assess knee function.Results Rasmussen imaging score of the first plain radiograph after operation:excellent in 7 cases,good in 19 cases,fair in 2 cases,with a score of 9-18,with an average score of(15.32±2.08).24 cases achieved bone healing within 6 months,and 4 cases had delayed healing.Healed after iliac bone grafting,the healing rate was 100.00%.At the final follow-up,the knee HSS scores were excellent in 7 cases,good in 15 cases,fair in 4 cases,and poor in 2 cases,with scores ranging from 47 to 93,with an average score of(78.81±9.41).Conclusion The application of external fixator for open SchatzkerⅤandⅥtype tibial plateau fractures has good quality of reduction and healing,which can effectively improve the knee joint function with satisfactory effect.
