温通三焦穴位埋线联合康复训练治疗脑卒中后肩手综合征Ⅰ期的效果

Effect of Wentong Sanjiao acupoint catgut embedding combined with rehabilitation exercise training in the treatment of shoulder hand syndrome in stage I after stroke

目的:评价温通三焦穴位埋线联合康复训练治疗脑卒中后肩手综合征I期的效果。方法:将72例脑卒中后肩手综合征I期患者随机分为穴位埋线组(36例)和对照组(36例)。穴位埋线组中男性22例,女性14例,平均年龄(67.00±4.18)岁;对照组中男性26例,女性10例,平均年龄(66.33±4.20)岁。穴位埋线组采用外关、阳池、腕骨、合谷、曲池、肩髃和肩井穴埋线法结合常规康复运动训练,对照组采用对应穴位常规针刺法结合常规康复运动训练。穴位埋线组疗程20天,对照组疗程14天。分别采用视觉模拟量表(VAS)评分、水肿分级评分、简式Fugl-Meyer量表(FMA)评分和Barthel指数评定两组患者治疗前后疼痛程度、水肿情况、上肢关节被动活动范围、上肢及腕手关节运动功能、和日常生活活动的功能状态。结果:两组患者治疗后,VAS评分、水肿分级评分、简式FMA评分及Barthel指数与治疗前相比均有明显改善(P<0.05),但穴位埋线组的改善效果均优于对照组(P<0.05)。穴位埋线组患者的治疗满意度评分明显优于对照组(P<0.05)。结论:穴位埋线联合康复运动训练对社区中老年脑卒中后肩手综合症I期患者有较好疗效,值得在社区中推广应用。

Objective:To evaluate the effect of Wentong Sanjiao acupoint catgut embedding combined with rehabilitation exercise training in the treatment of shoulder hand syndrome in stage I after stroke.Methods:Seventy-two patients with shoulder hand syndrome in stage I after stroke were randomly divided into an acupoint catgut embedding group with 36 cases and a control group with 36 cases.In the acupoint catgut embedding group,there were 22 males and 14 females with an average age of(67.00±4.18)years;in the control group,there were 26 males and 10 females with an average age of(66.33±4.20)years and the acupoint catgut embedding group was treated with acupoint catgut embedding at waiguan,yangchi,wangu,hegu,quchi,jianyu and jianjing acupoints combined with conventional rehabilitation exercise training.The control group received conventional acupuncture at the corresponding acupoints combined with regular rehabilitation exercise training.The acupoint embedding group was treated for 20 days and the control group for 14 days.Visual analog scale(VAS)score,edema grading score,simplified Fugl-Meyer scale(FMA)score and Barthel index were used to evaluate the pain degree,edema,passive range of upper limb joint movement,upper limb and wrist and hand joint movement function and daily activities of the patients in the two groups before and after treatment.Results:After treatment in the two groups,the VAS score,edema grading score,simplified FMA score,and Barthel index were significantly improved compared with before treatment(P<0.05),however,the improvement effect in the acupoint embedding group was better than that in the control group(P<0.05).The treatment satisfaction score in the acupoint-embedding group was significantly better than that in the control group(P<0.05).Conclusion:The acupoint catgut embedding combined with rehabilitation exercise training has better curative effect on patients with shoulder and hand syndrome in stage I after stroke in the community,which is worth promoting in the community.
