沙美特罗/丙酸氟替卡松联合缩唇呼吸治疗尘肺病合并慢性阻塞性肺疾病的效果观察

Effect observation of salmeterol/fluticasone propionate combined with lip shrink breathing in treatment of pneumoconiosis with chronic obstructive pulmonary disease

目的观察沙美特罗/丙酸氟替卡松联合缩唇呼吸治疗尘肺病合并慢性阻塞性肺疾病(COPD)患者的临床效果。方法选择邻水县人民医院呼吸内科和安岳县人民医院呼吸内科2017年1月-2018年12月收治的尘肺病合并COPD患者100例,根据随机数字表法分为2组各50例,对照组实施缩唇呼吸训练,观察组实施沙美特罗/丙酸氟替卡松联合缩唇呼吸训练治疗。比较2组的临床疗效、症状缓解时间、肺通气功能及血气分析。结果观察组临床总有效率为98.00%高于对照组的84.00%(P<0.05),咳嗽、咯痰及呼吸困难症状缓解时间均短于对照组(P<0.05)。治疗后2组FEV1、FEV1/FVC均高于治疗前,且观察组均高于对照组(P均<0.05)。治疗后2组Pa O2高于治疗前,Pa CO2低于治疗前,且观察组变化幅度大于对照组(P均<0.05)。结论沙美特罗/丙酸氟替卡松联合缩唇呼吸训练治疗尘肺病合并COPD可有效提高患者的临床疗效,加快症状缓解,改善呼吸功能。

Objective To observe the clinical effects ofsalmeterol/fluticasone propionate combined with lip shrink breathing in treatment of pneumoconiosis with chronic obstructive pulmonary disease(COPD).Methods 100 patients with pneumoconiosis and COPD treated of Department of Respiratory Medicine in Linshui People’s Hospital and Anyue County People’s Hospital from January 2017 to December 2018 were selected.According to the random number table method,they were randomly divided into 2 groups of 50 cases each.The control group received lip shrink training and the observation group received salmeterol/fluticasone propionate combined with lip shrink training.The clinical efficacy,symptom relief time,pulmonary ventilation function index and blood gas analysis index of the two groups were compared.Results The total clinical effective rate in the observation group was 98.00%higher than 84.00%in the control group(P<0.05).The cough,expectoration and dyspnea symptoms relief time of the observation group were all shorter than those of the control group(P<0.05).After treatment,FEV1,FEVF1/FVC in the two groups were higher than before treatment,and FEV1,FEV1/FVC in the observation group were higher than those in the control group(P<0.05).After treatment,Pa O2 was higher in the two groups than before treatment,Pa CO2 was lower than before treatment,and the change in the observation group was greater than that in the control group(P<0.05).Conclusion Salmeterol/fluticasone propionate combined with lip-reducing breathing training for pneumoconiosis combined with COPD can effectively improve the clinical efficacy of patients,accelerate the relief of symptoms,and improve respiratory function.
