泰安地区16648例妊娠妇女不规则抗体筛查分析

Analysis of 16648 pregnant women with irregular antibody screening in Taian

目的通过检测16648例妊娠妇女患者的不规则抗体及抗体鉴定,分析其构成及临床意义。方法收集泰安市中心医院自2015年1月至2018年12月妊娠妇女16648例,应用微注凝胶法对样本进行不规则抗体筛查,对不规则抗体阳性的结果进行抗体鉴定。结果在16648例标本中,共计检出阳性结果174例,不规则抗体阳性检出率为1.05%,同种特异性抗体149例,主要为Rh血型系统(106例,占60.92%),MNS血型系统(34例,占19.54%),Lewis血型系统(17例,占9.77%),P血型系统(2例,1.15%)。非特异性抗体主要为自身抗体(4例,2.29%)和其他未鉴定出的抗体(11例,6.32%)。无妊娠史妇女不规则抗体阳性0.61%(27/4400),有一次妊娠史妇女不规则抗体阳性率0.77%(14/1822),有两次妊娠史妇女不规则抗体阳性率1.20%(68/5644),有三次及以上妊娠史妇女不规则抗体阳性率1.36%(65/4782)。结论泰安地区妊娠妇女的红细胞血型不规则抗体主要以Rh血型系统为主,多次妊娠史是产生不规则抗体的主要原因之一。应重视妊娠妇女患者不规则抗体筛查结果,改进输血策略,提高临床输血安全。

Objective:To analyze the constitution and clinical significance of the irregular antibodies and antibodies in 16648 pregnant women.Methods:16,648 pregnant women from Taian City central hospital from January 2015 to December 2018 were collected.The samples were screened for irregular antibodies by microgel injection,and the positive results of irregular antibodies were identified by antibody.Results:among the 16,648 samples,a total of 174 were positive,the positive rate of irregular antibody was 1.05%,and 149 were homo-specific antibodies,mainly Rh blood group system(106 cases,60.92%),MNS blood group system(34 cases,19.54%),Lewis blood group system(17 cases,9.77%),and P blood group system(2 cases,1.15%).Non-specific antibodies were mainly autoantibodies(4 cases,2.29%)and other unidentified antibodies(11 cases,6.32%).Irregular antibody positive rate was 0.61%(27/4400)in women with no history of pregnancy,0.77%(14/1822)in women with one history of pregnancy,1.20%(68/5644)in women with two pregnancies,and 1.36%(65/4782)in women with three or more pregnancies.Conclusion:Rh blood type system is the main source of the irregular antibodies in the pregnant women in taian area.Attention should be paid to the irregular antibody screening results of pregnant women and the improvement of transfusion strategy to improve the safety of clinical transfusion.
