宫颈上皮内瘤变患者行宫颈锥切术后病变残留的危险因素分析

Analysis of risk factors for residual lesions in patients with cervical intraepithelial neoplasia after cervical conization

目的分析宫颈上皮内瘤变(CIN)患者宫颈锥切术后病变残留的危险因素。方法于2015年1月至2019年8月在我院接受宫颈锥切术治疗的CIN患者中随机选择70例术后病变残留的CIN的患者(观察组)与60例术后无病变残留的CIN患者(对照组)。回顾性分析两组患者的临床资料,分析CIN患者宫颈锥切术后病变残留的危险因素。结果多因素Logistic回归分析结果显示,年龄、绝经、吸烟史、术前病理分级、病变累及点数量、宫颈转化区分型、术后HPV持续感染均是CIN患者宫颈锥切术后病变残留的独立危险因素(P<0.05)。结论CIN患者宫颈锥切术后受到年龄、绝经、吸烟史、术前病理分级、病变累及点数量、宫颈转化区分型、术后HPV持续感染的影响,易发生病变残留,建议临床根据其危险因素采取针对性干预措施。

Objective To analyze the risk factors for residual lesions in patients with cervical intraepithelial neoplasia(CIN)after cervical conization.Methods From January 2015 to August 2019,70 cases of CIN patients with residual lesions after cervical conization(observation group)and 60 cases of CIN patients without residual lesions after cervical conization(control group)were randomly selected from CIN patients undergoing cervical conization in our hospital.The clinical data of patients in the two groups were analyzed retrospectively,and the risk factors of residual lesions of CIN patients after cervical conization were analyzed.Results The multivariate Logistic regression analysis showed that age,menopause,smoking history,preoperative pathological grade,number of lesion involved points,cervical transformation type and postoperative persistent HPV infection are independent risk factors for residual lesions in patients with CIN after cervical conization(P<0.05).Conclusion The CIN patients are prone to residual lesions after cervical conization under the influences of age,menopause,smoking history,preoperative pathological grade,number of lesion involved points,cervical transformation type and postoperative persistent infection of HPV.It is suggested to take targeted intervention according to the risk factors.
