股骨近端锁定钢板与空心钉两种内固定方法治疗不稳定Pauwels股骨颈骨折的临床效果

Clinical effect of two internal fixation methods of proximal femoral locking plate and hollow nail in the treatment of unstable Pauwels femoral neck fracture

目的探究股骨近端锁定钢板与空心钉两种不同内固定方法治疗不稳定Pauwels股骨颈骨折的临床效果。方法回顾性分析2014年12月至2017年12月于我院接受治疗的102例不稳定Pauwels股骨颈骨折患者的临床资料,根据手术方案将患者分成观察组(54例,采用股骨近端锁定钢板内固定治疗)和对照组(48例,采用空心钉内固定治疗)。比较两组的手术情况、临床疗效及术后并发症发生情况。结果观察组治疗后Harris功能评分高于对照组,术中出血量和VAS评分低于对照组(P<0.05)。观察组治疗总有效率高于对照组(P<0.05)。两组术后并发症总发生率比较,差异无统计学意义(P>0.05)。结论股骨近端锁定钢板较常规内固定空心钉具有更好的疗效,术后并发症发生率较少,可以作为替代空心钉内固定治疗方法的安全选择。

Objective To investigate the clinical effect of two internal fixation methods of proximal femoral locking plate and hollow nail in the treatment of unstable Pauwels femoral neck fracture.Methods The clinical data of 102 patients with unstable Pauwels femoral neck fracture treated in our hospital from December 2014 to December 2017 were retrospectively analyzed.According to the operation plan,the patients were divided into observation group(54 cases,treated with proximal femoral locking plate internal fixation),and control group(48 cases,treated with hollow nail internal fixation).The operation,clinical efficacy and postoperative complications in the two groups were compared.Results After treatment,the Harris function score of the observation group was higher than that of the control group,while intraoperative bleeding volume and VAS score were lower than those of the control group(P<0.05).The total effective rate of treatment in the observation group was higher than that in the control group(P<0.05).There was no significant difference in the total incidence of postoperative complications between the two groups(P>0.05).Conclusion Proximal femoral locking plate has better curative effect than conventional hollow nail internal fixation,and with less incidence of postoperative complications,so it can be used as a safe alternative to hollow nail internal fixation.
