彩色多普勒超声在非赘生性囊肿和囊腺瘤中的诊断价值

Diagnostic value of color Doppler ultrasound in non-neoplastic cyst and cystadenoma

目的探讨彩色多普勒超声在非赘生性囊肿和囊腺瘤中的临床诊断价值。方法回顾性分析我院2018年1月至2019年6月收治的经手术病理证实的非赘生性囊肿(52例)和囊腺瘤(52例)患者的临床资料。患者均采用彩色多普勒超声进行检查诊断,分析非赘生性囊肿和囊腺瘤的声像图特征,并比较彩色多普勒超声对其的诊断符合率。结果非赘生性囊肿声像图特征:黄素囊肿边界清晰,呈圆形或椭圆形,囊壁偏薄,其内部为液性暗区;巧克力囊肿边界不清晰,呈圆形或椭圆形,囊壁较厚,囊内有较多点状回声;多囊卵巢两侧卵巢增大,表面凹凸,不平整,内部存在较多的小暗区。囊腺瘤声像图特征:浆液性囊腺瘤轮廓清晰,呈圆形或椭圆形,囊壁偏薄,囊内部无回声;黏液性囊腺瘤轮廓清晰,呈圆形或椭圆形,囊内呈多房结构,房室大小不一。彩色多普勒超声对非赘生性囊肿的诊断符合率为94.23%(49/52),其中黄素囊肿15例,巧克力囊肿23例,多囊卵巢11例;彩色多普勒超声对囊腺瘤的诊断符合率为96.15%(50/52),其中浆液性囊腺瘤27例,黏液性囊腺瘤23例;两种肿瘤的彩色多普勒超声及手术病理结果无显著差异(P>0.05)。结论非赘生性囊肿和囊腺瘤在临床特征和表现方面存在较多的相似之处,通过彩色多普勒超声诊断检查,诊断符合率较高。

Objective To explore the clinical diagnostic value of color Doppler ultrasound in non-neoplastic cyst and cystadenoma.Methods The clinical data of patients with non-neoplastic cyst(52 cases)and cystadenoma(52 cases)confirmed by surgical pathology in our hospital from January 2018 to June 2019 were retrospectively analyzed.All patients were examined and diagnosed by color Doppler ultrasound.The sonographic characteristics of non-neoplastic cyst and cystadenoma were analyzed,and the corresponding diagnostic coincidence rate of color Doppler ultrasound in which were compared.Results The sonographic characteristics of non-neoplastic cysts:the flavin cyst has a clear boundary,round or oval shape,thin cyst wall,and a liquid dark area inside;the chocolate cyst has unclear borders,round or oval shape,the cyst wall was thicker,and there were more punctate echoes in the cyst;the polycystic ovary has enlarged ovaries on both sides,concave and convex surface,unevenness,and more small dark areas inside.The sonographic characteristics of cystadenoma:the serous cystadenoma has clear contour,round or oval shape,thin cyst wall and no echo inside the cyst;the mucinous cystadenoma has clear contour,round or oval shape,with multilocular structures in the cysts and varying atrioventricular sizes.The diagnostic coincidence rate of color Doppler ultrasound in non-neoplastic cysts was 94.23%(49/52),including 15 cases of flavin cyst,23 cases of chocolate cyst,and 11 cases of polycystic ovary;the diagnostic coincidence rate of color Doppler ultrasound in cystadenoma was 96.15%(50/52),including 27 cases of serous cystadenoma and 23 cases of mucinous cystadenoma;there were no significant differences in the diagnosis results of the two tumors between color Doppler ultrasound and surgical pathology(P>0.05).Conclusion Non-neoplastic cyst and cystadenoma have more similarities in clinical features and manifestations,and have higher diagnostic coincidence rate by color Doppler ultrasonography.
