摘要
目的应用3.0T磁共振成像的同步非对比剂血管成像和斑块内出血成像(SNAP)对颈动脉斑块成分特异性研究,进而得出对斑块内成分显示最佳的图像。方法选取经颈动脉超声检查有斑块形成的病人52例,进行双侧颈动脉HRMRI检查,使用SNAP中Ref、CR、IR三组图像联合应用Vessel Explore分析软件对颈动脉斑块内的成分进行判别。结果斑块内出血、钙化成分在SNAP序列中Ref、CR、IR三组图像显示之间存在统计学差异(P<0.05);颈动脉斑块所在层面斑块内钙化成分与最大管壁厚度具有相关性(r=0.861,P=0.000);颈动脉斑块所在层面斑块内出血成分与最大管壁厚度具有相关性(r=0.799,P=0.000);颈动脉斑块所在层面斑块内脂质成分与血管面积具有相关性(r=0.874,P=0.000),斑块内脂质成分与管壁面积具有相关性(r=0.856,P=0.000)。结论SNAP序列中Ref图像对钙化显示最佳,CR图像对斑块内出血显示最佳,斑块内出血、钙化均与最大管壁厚度有关,脂质与血管面积、管壁面积有关。
Objective Using 3.0T MR synchronous non contrast vessel imaging and plaque bleeding imaging(SNAP)to study the component specificity of carotid plaque,and then get the best image to display the component of plaque.Methods Fifty-two patients with plaque formation were selected for hrmri examination of bilateral carotid arteries.The three groups of images of Ref,CR and IR in SNAP were used together with the analysis software of Vessel Explore to distinguish the components in carotid plaque.Results There were statistical differences in the images of Ref,CR and IR in the SNAP sequence of bleeding and calcification components in the plaque(P<0.05).There was a correlation between calcification components in the plaque and the maximum wall thickness(r=0.861,P=0.000).There was a correlation between bleeding components in the plaque and the maximum wall thickness(r=0.799,P=0.000).There was a correlation between the lipid composition and vascular area(r=0.856,P=0.000),and between the lipid composition and vascular area(r=0.856,P=0.000).Conclusion In SNAP sequence,Ref image is the best to show calcification,CR image is the best to show bleeding in plaque.Bleeding and calcification in plaque are related to the maximum wall thickness,while lipid is related to vascular area and wall area.
